lOB'Heaith Care Plus is a group rnedicloim insurance offered to the account
holders of lOB aged between 18-65 years. The scheme covers the family
comprising account holders, spouse and two dependent children with a
nominal premium. Even parents of account holder are covered for additional
premium. No medlcol examinations. I^rsonal accident death covered for additional
premium.

Eligibility: All customers of the Bank maintaining either a S.B. account or Current
account are eligible to optfor the scheme.
Amount of premium: The premium amount depends on the sum Insured.
Insurance coverage: Hospltallsatlon expenses for Illness/diseases contracted and
infurysustained bythe Insured Is covered bythepoiicyuptothesum Insured.
