Park, One Finetech, Megaworld Boulevard, Mandurriao, Iloilo City. Hence my application for the position
of Project-Based Occupational Health Physician. Earlier in my career, I have served as a company
physician at Healthway Medical Clinics and Active One Medical Clinics in Manila from Year 2013-2016. I
was a Medical Examiner at VNB Medical Clinic of the Land Transportation Office, Imus, Cavite in Year
2014-2016. I have been on the private medical practice from Year 2017-2020. From the year 2021-2022,
I worked as a Medical Officer and Iloilo Provincial Hired Vaccinator screening patients for COVID-19
Vaccination. At present, I am a Medical Examining Physician at DAL Medical Clinic of the Land
Transportation Office, Brgy. Quintin Salas, Jaro, Iloilo City.

I am a Licensed Physician and Career Service Professional. I also hold certification in “Basis Course in
Occupational Medicine” conducted by the Philippine College of Occupational Medicine, and PCOM 1 st
Virtual and Annual National Convention.

Throughout my 9- year healthcare, I have earned exemplary ratings from patients, colleagues and
consultants for my clinical skills, quality of care, medical judgment and teamwork. Likewise, I am deeply
