Introduction
As part of the need assessment for the proposed position at your department an evaluation of the tasks conducted by the
current staff is also required. Each of the relevant staff members is required to fill in this form with the approval of the HOD
and provide documented evidence of the tasks performed wherever possible.
Please list the tasks performed by you as per your job description during the last six months. For each of the
responsibilities assigned to you, a brief description, the frequency of that responsibility during the last six months,
and whether there is any documented evidence are to be mentioned. If there is any documented evidence, please
send a soft/hard copy separately.
You may also add to the rows of the table and mention those tasks performed that you consider important but are not
mentioned in your job description.
