CERTIFICATION

This is to certify that Hon. _______________ is a duly elected and proclaimed

Sangguniang Kabataan Chairperson/Member of Barangay ___________, Municipality of San
Luis, Province of Pampanga.
This is certification is issued pursuant to Section 16 paragraph a.2 declaring the
exemption of SK officials from taking the National Service Training Program- Civic
Welfare training Service (NSTP-CWTS) subjects. In lieu thereof, concerned SK Official’/s
shall submit the following to the University, to wit:
1. Monthly accomplishment report with photographs,
2. Other documentation of their participation in the implementation of program,
projects and activities as outline by ABYIP.

Absence of abovementioned reports and documentation or a finding to the contrary

upon verification of submitted report, will disqualify the concerned SK official from this
privilege.
