Re: Request for authority to sign the Memorandum of Understanding.

Dear members of the Sangguniang Panlungsod,

Greetings!

This is to formally request for a resolution authorizing the City Mayor to enter and sign the
Memorandum of Understanding (MOU) between the City Government of Panabo (City for
brevity) and PEEDO – Davao del Norte Hospital – Carmen Zone (PEEDO for brevity)
concerning the hospital and ambulance usage as a requirement for the renewal of the License to
Operate by the City Health Office of the Emergency Clinic and Safe Birthing facility.

Attached herewith is a copy of the proposed Memorandum of Understanding, as well as the

endorsement from the City Legal Office.

Respectfully endorsed to Hon. City Mayor Jose E. Relampagos, the attached copy of the
proposed Memorandum of Understanding (MOU) between the City Government of Panabo (City
for brevity) and PEEDO – Davao del Norte Hospital – Carmen Zone (PEEDO for brevity)
concerning the hospital and ambulance usage as a requirement for the renewal of the License to
Operate by the City Health Office of the Emergency Clinic and Safe Birthing facility.

After a review of the proposed Memorandum of Understanding, our Office found that the same
is in order. Hence, the office recommends that it be endorsed to the Sangguniang Panlungsod for
consideration.
