1.3 Observations
1. The original Ship Station License certificate was not available for verification.
However, the copy certificate was sighted at the time of this inspection.
2. The original Exemption statement for Ballast Water Management was not available
for verification. However, the copy certificate was sighted at the time of this
inspection.
3. The shore contact list in SOPEP manual was not updated.
4. The lashing inventory list attached in the Cargo Securing Manual was not updated.
5. The joint weld of two (2) units of grating floor located at the aft bridge was found
to be deteriorated.
6. Three (3) units of lifejacket lights located at monkey island were missing.
7. The fire detection panel was faulty at the time of this inspection. System error on
Zone 8 and 10.
8. The stair footstep from the navigation deck to the bridge deck was found
deteriorated (3 steps from the top).
9. The staircase footstep from the top of the bridge deck to the storage winch was
found to be deteriorated with a hole (step No.1).
10. The wire mesh and casing plate of the port side engine room exhaust blower sighted
to be deteriorated Tension meter calibration certificate was not available.
11. The wood sheathing of port side rescue zone door was found to be missing.
12. Low installation alarm of 440 voltage was activated on the main switchboard panel
from the AE No.2
13. AE#1 and AE#2 were unable to be automatically synchronized to share the load
since the system was provided with the changing-over switch.
14. AE# 2 was unable to be started remotely in the engine control room due to the
automatic starting system being found to be defective and in addition, a system
modification with the installation of switching-over.
15. It was observed that the ceiling in the recreation room was found partly deteriorated
with moderate rust.

16. A sign of gas leak was identified at the port generator.
17. The cap of the radiator coolant tank on the port generator was missing.
18. The following safety device alarms of both main engines were found defective during
functional testing:
a. ME FW pressure high temperature
b. ME oil mist detector
19. Several safety device alarms of both main engines were yet to be tested at the time
of this inspection due to constrained time as follows:
a. Thrust bearing temperature gear
b. Lube oil temperature
c. Exhaust gas temperature
d. Lub oil Pressure
e. Electric governor failure
20. It was noted that both engines were hardly to start at the time of this inspection.
The unit needs 3-4 times to start the engine. It is recommended that the fully
operational before on-hire.
21. The LO purifier pumps No.1 and No. 2 were found to be defective at the time of the
function test.
22. FO purifier was found to be defective.
23. The pressure gauge of FO transfer pump was defective.
24. The pressure gauge of the drill water pump was found defective.
25. Last lube oil analysis was not available for verification as the following:
a. Stern Tube
b. FIFI Gearbox
c. Anchor Handling and Towing Winch
d. Deck Crane
e. Anchor Windlass
f. Emergency Generator
g. Controllable Pitch Propeller (CPP)
26. Frequency indicators in the emergency generator were found to be defective. The
unit reading is 56-57 Hz.
27. The last destructive test certificate of the following wire was missing main towing
wire, work wire, and spare tow wire.
28. The fire damper for the emergency generator room was not operational at the time
of this inspection.
29. The exhaust blower for the galley was deteriorated.
30. The lip of chain locker spurling pipe was found to be heavy corroded
and deteriorated Port side embarkation ladder was yet to be installed.
31. Hydraulic pressure for both capstans was not fully operational since both units could
not be tested simultaneously.
32. Fanbeam (PRS) was not operational.
33. The water leak was sighted on all flushing systems (water leak on the handle)
except hospital and common toilet.
34. Several tiles in the 2nd Engineer toilet were found deteriorated.
35. The refrigerator unit in the Chief Engineer’s cabin was found defective.
36. AC blower was not operational in the hospital.

37. Vinyl floor sheet in the mess room was found deteriorated 20x 70 cm
38. Two (2) out of six (6) hot plate stoves were found defective.
39. The connecting hose of the dispersant spray boom was found missing.
40. The ampere meter of starboard ME hydraulic lube oil pump was found hunting.
41. Port and starboard anchor chain found to be degraded between shackles #7 and
#8 from the anchor end.
42. Freezer compressor was not fully operational since the room temperature could not
reach -10 degrees.
43. Freezer and chiller compressor No.2 were yet to be tested due to time-constrained.
44. The portable AC in the ECR was found to be defective.
45. The anchor handling winch was not fully operational where the clutch was hardly to
be engaged and disengaged.
46. The suction and delivery valve of FIFI pump was not operational.
47. Gland packing of the port side FIFI monitor was found defective. A sign of water
leakage was sighted from the gland packing.
48. CO2 system was yet to be tested due to time constrained.
49. Air quality analysis of the oxygen refill compressor was not sighted.
50. The RADAR service report for the replacement of the magnetron was not sighted.
51. The last pressure test certificate for all flexible cargo hoses was not available.
52. The dimmer light of the magnetic compass was found defective.
53. All fire hoses were unable to be tested due to time constrained.
54. The pressure of fresh water hydrophone standby pump was not sufficient (reading
2 kg). It is recommended to adjust the pump pressure to the normal level.
55. A sign of water leakage sighted on the starboard AE sea water cooling pump.
56. A sign oil leakage on the starboard AE engine.
57. A sign water leakage on port main engine the safety device.
58. Towing pins was found dry and lack of maintenance.
59. Internal tank inspection
a. Forepeak
i. Aft bulkhead found indentation at frame 91 location approximately
50 cm from the bottom with approximately measurement 300 cm x
5 cm x 200 cm.
ii. Aft bulkhead found indentation at frame 91 location approximately
400 cm from the bottom with approximately measurement 200 cm
x 5 cm x 180 cm.

centre bow steel plate.

several area.

2. DEFINITIONS

2.1.1 The vessel's description, weights, measurements, capacities, status of certificates


and report contents noted herein are as abstracted from plans and documents
presented/made available or otherwise informed at the time of the survey onboard
by the operating staff at time of inspection only.
2.1.2 It is to be clearly understood that the conditions and /or states of items reported
upon herein below, are strictly the opinions of the report authors and that those
opinions fairly reflect the findings made during the course of the survey. All
statements of condition are made in comparison with new condition.
2.1.3 The following are used to clearly describe items inspected as being "Good", "Fair",
"Poor" or "Unsatisfactory" condition:
Good: Unimpaired condition without significant wear or deviation
from original strength and operating efficiency. No
maintenance or repair required.
Fair: Condition with wear and tear and other deficiencies of minor
nature not requiring correction or repair.
Poor: Condition in which the adequacy of strength and/or
operational efficiency is marginally below acceptable limits,
or is in doubt. Remedial action is required.
Unsatisfactory: Condition of undoubtedly inadequate strength or operational
efficiency. Immediate extensive repair or renewal required to
reinstate serviceability.
2.1.4 The following terms are used to describe the acceptability or operational status of
a given element/item/question in relation to function, acceptability against an
intended standard or procedural compliance:
Satisfactory: Fully acceptable for the intended purpose, compliant with an
intended standard or expectation, unimpaired status without
deviation from its original purpose or function; fully
operational status and fit for the intended purpose without
need for remedial action.
Unsatisfactory: Status not fit for the intended purpose or in a non-
operational/functional status, non-compliant with an intended
standard or expectation. Immediate remedial action required
to reinstate to an acceptable standard or fully-functional status
based upon original and intended purpose.

3. VESSEL DESCRIPTION

SMS SONET is an anchor handling towing supply vessel built by Pax Ocean Shipyard
Singapore Pte Ltd, Singapore in 2005. She is presently registered in Jakarta, Indonesia
and classed by American Bureau of Shipping with Class Notation: ABS, A1, Towing
Vessel, Fire Fighting Vessel Class 1, Offshore Support Vessel, AH, Ⓔ, AMS.

She is provided by two MAK 8M25 diesel main engines of 3,540 bhp each (total 7080
bhp) driving a pair of controllable pitch propellers in fixed kort nozzles. It was noted that
both engines were hardly to start at the time of this inspection. The unit needs 3-4 times
to start the engine as well as the safety device alarms of both main engines were found
defective during functional testing, i.e.,ME FW pressure high temperature and ME oil mist
detector.

One (1) 590 kW electrical bow thruster is installed which reportedly could provide 9
tonnes thrust. The bow thruster sighted working satisfactorily during limited sea trials
at PT. Vesinter Shipyard.

Electrical power is provided by two (2) Caterpillar 3408 diesel generators each having
350 kW output @ 440V, 3ph, 60Hz, and two (2) AVK shaft generators each of 800 kW
output @ 440V, 3Ph, 60Hz. There is one (1) Caterpillar 3304 DITA emergency generator
of 116 kW installed on board.

The navigation and communication systems are in compliance with SOLAS/Class


Society’s requirements. All navigation and communication were checked where
practicable and all appeared in satisfactory condition.

The lifesaving and firefighting equipment are generally noted in satisfactory condition
with valid service dates.

The vessel is fitted with Brattvaag SL225W/2BSL225W water-fall double drum


towing/anchor handling winch system, which consists of a towing drum and a working
drum, each rated at 496,000 lbs / 220 tonnes maximum pull on the first layer with 275
tonnes brake holding power. The towing/ anchor handling winch system was function
tested. The 58 mm diameter main towing wire and work wire were noted in satisfactory
condition as seen on outer layer. Similar size spare towing wire was stowed on the spare
reel.

Two (2) 10 tonnes tugger winches and two (2) 5 tonnes capstans are installed on the
main deck. One (1) electro-hydraulic 300 tonnes SWL Karm Fork and a pair of 300 tonnes
SWL towing pins were installed at the aft main steel deck. Karm forks, towing pins,
tuggers and capstans were tested and found in working condition. Deck Crane SWL 1.9
tonnes fitted on board with satisfactory condition.
