Notes 0

As a distributor of medical products, we are obligated to provide our systems with a CE

label, which verifies with proof that the basic requirements have been met. This document
shows on which system components the system CE label is attached.
If a component which has the system CE label has to be replaced in a system, it is
required to attach a duplicate of the original system CE label on the replaced component.

Purpose of this Document 0

On which components the system CE label is attached is shown for all AX systems that
are currently being shipped, and if needed, how a duplicate system CE label can be
ordered.

Ordering Information for a Duplicate System CE Label 0

Place an order as follows:

• print out the form “(AX00-000.841.03 / External Request for Self-adhesive System ID
Labels)”
• record the data from the original system CE label (Model No. Serial No. CE labeling
etc.)
• fill out the form
