AUTHORIZATION LETTER

I, the undersigned, authorize my mother, Ms. Alma G. Rabasano, of legal age,

to claim my passport which is tentatively scheduled to be released today. I
am not able to present myself due to time constraints at work.

To support my claim, together with the original copy of the electronic receipt
(NO. 23500691002), I have appended photocopies of my valid IDs with 3
specimen signatures which serve as a proof that I allowed the bearer of this
letter to claim the said document on my behalf.
