N O T I C E

Company records and reports show that you have been remiss in your duties and responsibilities to
the Company by committing the following acts that are inimical to the interest of the Company and its
customers/clients, particularly in the following instances:

On January 9, 2024, you failed to substitute Teacher Marga’s class on time despite being
informed prior to her absence. So, I instantly assigned Teacher Jay Ann to do the substitution
instead, while Sir Al reprimanded you about this incident.

On January 10, 2024, you failed to be in your class on time. During the first class, I assigned
you in the Infirmary Room to look after an unwell student since you have no load during this time.
During the second period, we could not find you in your class until minutes later, Sir Al found you
over napping in the Infirmary Room, then reprimanded you on the same.

Please explain in writing, within 5 working days upon receipt hereof, why you should not be
subjected to appropriate disciplinary action in connection with the foregoing acts and misconduct.
Failure to respond within the period given will be construed as a waiver of your right to be heard and
the investigation on the foregoing matter will proceed. In the event that after due investigation the
imposition actions is warranted, same shall be made known to you.

Furthermore, considering the nature and gravity of the offense charged and the sensitivity of
the nature of the work you are currently occupying, the Company has the prerogative to impose
appropriate penalties or worse, termination of employment.
