THREAT AND ERROR MANAGEMENT
THREATS are events that occur outside the influence of the flight crew.
ERRORS are flight crew actions or inactions that cause deviation from SOPs and reduce safety.
THREATS come “at” the crew, while ERRORS come “from” the crew
UNDESIRED AIRCRAFT STATE (UAS) Any A/C: position, speed, attitude, condition, configuration or
trend away from safe operations that reduces safety margins often a result of mismanaged errors.
THREAT AND ERROR MANAGEMENT APPLIES TO ALL PHASES OF FLIGHT
THREATS, ERRORS and UAS must be IDENTIFIED and MANANGED
THREATS
IDENTIFY THREATS MANAGE THREATS
Environmental Threats
Weather: FOLLOW THE SOPS.
Thunderstorms, turbulence, poor visibility, wind shear, Use CRM and maintain good SA.
tailwind, icing conditions, IMC. Complete briefings.
ATC: o Apply avoid TSRA checklist (if required)
Tough-to-meet clearances/restrictions, reroutes, language o Apply de-icing checklist (if required)
difficulties, controller errors. Develop contingency plans for threats.
Airports: VERBALIZE, VERIFY, MONITOR
Poor signage, faint markings, runway/taxiway closures, Verbalization by both pilots “verifies” a
INOP navigational aids, poor braking action, contaminated shared understanding of what is
runways/taxiways, radio congestion. expected and then both pilots “monitor”
Terrain: High MSA/MEA/MORA/MOCA. to ensure strict adherence to all
Traffic: Traffic congestion, TCAS TA/RA. instructions.
Airline Threats Workload assignment.
Aircraft Malfunctions, Airline ops pressure, Respect PF/PM roles and make standard callouts.
Cabin events, Dispatch /Paperwork, Ground/Ramp Communicate plans and intentions to:
handling, Ground maintenance, EFB/manual/ charts o The other pilot
validation date o ATC or other appropriate resources
Internal Threats Limit being “heads down” at critical times.
Stress, Fatigue and Distractions
ERRORS
IDENTIFY ERRORS MANAGE ERRORS
Crew Errors (Human Errors):
Procedural errors, Communication errors, Resist errors
Proficiency errors, Decision errors. Resolve errors
CREW VIOLATIONS:
Intentional Noncompliance with SOPS. STOP CREW VIOLATIONS. FOLLOW THE SOPs.
UNDESIRED AIRCRAFT STATE (UAS)
IDENTIFY UAS MANAGE/RECOVER FROM UAS
Aircraft Upset: Loss of control, flight deviations, Recognize upset. Apply RECOVERY procedure
weather penetration, outside A/C limitations, Follow: memory items, abnormal-emergency
Aircraft handling: wrong weight and balance, procedures, and the SOPs.
wrong AC config, wrong FIR, wrong approach/ Monitor/Cross-check: make standard callouts
runway, unstable approach/landing. and respect PF/PM roles
Ground Navigation: Wrong taxiway/ramp/parking, Restore the airplane to safe operations and
taxi to wrong runway maintain good SA
