    Dear Engr. Arevalo:

    Please be notified that you are the declared Single Calculated Bidder (SCB) for the
    Construction of 3-Bedroom Staff House (Early Procurement Activity (EPA) for FY 2024) with a
    total calculated bid amount of EIGHT MILLION SEVEN HUNDRED NINETY-THREE
    THOUSAND, SIX HUNDRED NINETY-ONE AND 41/100 (P 8,793,691.41).

    As the declared SCB for the said procurement, may we request the submission of the following
    documents:

    A. Certified True Copy (CTC) of Post-Qualification documents

    Latest income and business tax returns filed and paid through the BIR Electronic Filing
    and Payment System (eFPS):
    a. 2023 Qtr 3 Income Tax Return with proof of payment;
    b. VAT Returns (2550Q) with proof of payment covering the months from July
    to September 2023 or Percentage Tax (2551Q) with proof of payment
    covering the months from July to September 2023, whichever is applicable.

    B. Original copies of the following for purposes of verification


    1. PhilGEPS Registration 5. BIR stamped received Annual Income Tax
    Certificate (Platinum Membership) Return and Audited Financial Statement
    2. DTI Certificate for FY 2022
    3. Mayor's Permit 6. PCAB License
    4. Tax Clearance 7. Authenticated copy of licenses of all key
    personnel
    C. Original copy of Detailed Unit Price Analysis (DUPA)
