AFFIDAVIT OF UNDERTAKING

I, MIRIAM A. GAYETA, of legal age, married, with residence and postal

address at Gayeta Store, Leo St., Brngy. Leonarda, Tuguegarao City, Cagayan after
being duly sworn to in accordance with law, hereby depose and say, THAT:

1. I entered into a loan agreement on March 20, 2024 with Ms. Myrna A. Bosi, a
resident of Zone 7, Paguirigan St., Caggay, Tuguegarao City;

2. I acknowledge that I have an outstanding principal loan to Ms. Bosi

amounting to Two Hundred Eighty Nine Thousand and 00/100 pesos (P 289,000.00);

3. I undertake to pay the said principal loan and its corresponding interest to Ms.
Bosi within thirty six (36) months in accordance with the herein attached schedule of
payments (Annex A);

4. I am willing voluntarily to carry on the obligation in relation to the aforesaid

undertaking; and

5. I am executing this affidavit of undertaking to attest to the truth of the foregoing

statement and for whatever legal purpose it may serve best.
