The purpose of this form is to provide the employee with the evaluation on their work based on
the service level rendered for the specific contract period by providing both with specific performance
objectives and standards; provides awareness to employees of the performance standards which is
applicable to their jobs.

Instructions:

Use the performance levels listed below that most accurately describes your employee’s

Excellent – exceeds work requirements and exceeds many expectations on the scope of the job.

Good – exceeds some expectations and scope of job. Accomplished job with minor problems

Needs Improvement (NI)– some job requirements were met
