AFFIDAVIT OF UNDERTAKING

I, JOSEPH ALLEN D. DE VERA, Filipino, of legal age, and presently employed /

enrolled at MAPUA UNIVERSITY, after having been duly sworn to in accordance with law,
hereby depose and say:

1. That I will conduct a research study entitled Preserving Roots, Building the Future:
Integrating Regionalism and Vernacular Architecture Approach for T’boli Cultural
Sustainability – A Proposed Cultural Village located at LAKE SEBU, SOUTH
COTABATO.

2. That the period of my research study will commence after the issuance of the
Certification Precondition (CP).

3. That I am willing to comply the required process/procedures of research and

documentation of Indigenous Knowledge Systems and Practices (IKSP) laid down under
Administrative Order No. 1, Series of 2012 of the NCIP;

4. That I, HEREBY UNDERTAKE to shoulder all administrative cost/expenses incidental

to the research activities and further UNDERTAKE that everything are in good faith and
will truly abide by this Guidelines and/or other requirements.

I am executing this Affidavit of Undertaking in order to attest to the truth of all the foregoing
statements and for whatever other legal purposes it may serve.
