METRO MANILA DEVELOPMENT SCREENING TEST

Description
The Metro Manila Developmental Screening Test (MMDST) is a simple, clinically useful tool used in
the early detection of children with serious developmental delays developed by Dr. Phoebe D.
Williams. It is a modification and standardization of the original Denver Developmental Screening
Test (DDST) by Dr. William K. Frankenburg and his associates. It was developed for health
professionals such as nurses, doctors, and other health workers so that slow development may be
recognized, referrals made, and appropriated therapy prescribed as possible. It is not an intelligent
but a screening instrument to determine whether a child’s development is within normal range.

MMDST is designed to detect developmental delay for children 2 weeks to 6 ½ years old. It evaluates
four aspects of development: personal-social, fine-motor-adaptive, language, and gross behavior.

Learning objectives:
After reading this module, the student nurse will be able to:
1. Discuss the purpose of MMDST
2. Describe the four aspects of development
3. Prepare the materials including the scoring sheet for administering MMDST
4. Identify the steps for administering MMDST
5. Describe scoring for MMDST Definition of terms
Age line – the line drawn from top to bottom of the test form based on the age of the child.
Age of the child – date of test minus the birth date of the child in years and
Months.
Aspect of development – the fours aspects of development that is measured in
MMDST. Screening test to determine any developmental delays
Fine motor Adaptive - tasks which indicate the child’s ability to see and use his hands to pick up
objects and to draw
Gross-Motor - tasks which indicate the child’s ability to sit, walk and jump
Language - tasks which indicate the child’s ability to hear, follow directions and to speak.
No Opportunity – the child has no chance of performing the task due to unavailability of resources. An
example is rides a tricycle.
Passed – the child was able to perform the task.
Personal-Social - tasks which indicate the child’s ability to get along with people and to take care of
himself
Refused – the child does not want to perform the task after three tries.
Screening - the presumptive identification of unrecognized disease through tests and examinations.
Test items – 105 tasks to be tested to the child which are divided into four sectors.

Aspects of development

MMDST can measure the four aspects of development (sectors in the test form) of a child:

1. Personal-Social
2. Fine-Motor Adaptive
3. Language
4. Gross-Motor

MMDST KIT

These materials should be followed as specified (Fig. 1):

Steps in Administering the test

A. Before administering the test


1. Calculate the child’s age
Calculate first the days, then the months, and then the years. Example: Date of test is June 10, 2020
and the birth date is March 5, 2017 (Fig 4). The age of the child is 3 years, 3 months and 5 days.

2. Draw the age line

All the items before and those crossing the age line should be tested. Write the
date of the test on top of the age line.

Adjustment for Prematurity

There is therefore a need to make adjustment for prematurity for children aged 2 years and
younger. Draw the age line based on the
adjusted age, not the chronological age.

3. Select the items to be administered.

In selecting items to be administered, keep in mind the following:


a. All items crossing the age line should be administered.
b. The child should have a minimum of three passes to the left of any failure. c. Each sector should
have at least three passes and three failures.

The letter R in the bar as shown in Fig, Only items with an R on the form
may be passed by report. However, whenever.

B. The Test Procedure

1. Preliminary Phase: Establish rapport with the mother/caregiver.


2. Make the child as comfortable as possible during the test.
3. The items to be administered are those through which the child’s chronological age line passes
unless there are obvious devations.
4. Generally, the test starts with:
A. Personal-Social sector.
B. Fine motor-adaptive.
C. Language sector.
D. Gross- Motor sector is the last.
5. If a child passes an item, a large letter “P” is written on the bar at the 50% passing point, “F”
designates a failure, and “R” designates refusal.
6. Note how the child adjusted to the examination – and how the child related to the parent, the
examiner, and the test materials.
7. To retest the child on the same form, use a different color pencil for scoring and age line.

C. After the test.

1. Interpret the findings.


Step 1 – Mark each delay by heavily shading the right end of the bar
Step 2 – Count the number of sectors that have 2 or more delays
Step 3 – Count the number of sectors that have 1 delay with no passes intersecting the age
line in the same sector
