    Greetings!
    I, Mark Andrew Tan, am writing this letter is to authorize my mother, Mrs.
    Myrna Yu Tan to collect the UMID Card applied for on December 13, 2019 at
    SSS OROQUIETA BRANCH.

    I am currently in Paranaque City and will be unable to travel to the City of


    Oroquieta, Misamis Occidental to personally collect such UMID card. Attached
    herewith is a copy of my valid government issued ID and the necessary
    information required for the collection or claim of the UMID. If you require
    further authentication you may ask my mother to verify my identification through
    a videocall.

    I would appreciate any kind of assistance you can offer my beloved mother with
    regard this matter.
