AFFIDAVIT OF LOSS

I, GRACE LYN T. DAYDAYAN, 18 years old, Filipino, single and a resident

of Brgy. Maslog, Sibulan, Negros Oriental after having been sworn to in accordance
with law, do hereby depose and say the following, to wit:

That I am a student of Saint Paul University Dumaguete, Dumaguete City for

the School Year 2019-2020;

That it was last week, I discovered that my attendance card was lost when I
tried to retrieve the same;

That I immediately conducted a search for the said attendance card and even
ask my parents and classmates as to whether or not they have seen my attendance
card, to which they answered in the negative;

Thus, despite serious efforts in trying to locate said attendance card all such
efforts proved unproductive and futile as the same could no longer be found and it is
for this reason that I now consider said attendance card as lost and irretrievable for
all legal intents and purposes;

That I am executing this affidavit freely and voluntarily in order to attest to

the veracity of the facts above-stated;
