AFFIDAVIT OF NO EMPLOYEE

I, MADELEINE C. TUDTUD, of legal age, Filipino, married, and a

resident of Blk. 1, Lot 60 Maya Maya, Lane Villa Del Rio Subdivision.
Barangay Babag 2, Lapu-Lapu City, Cebu, hereby state under oath:

1. That I am the Owner of FROLICS TRAVEL AND TOURS, a

business establishment with office address at Mardeca/MDC Ent.
Building, North Road, Barangay Labogon, Mandaue City;

2. That the establishment had no employees for the months of April

to May 2017, and August 2018. For the months of January to
February 2018, the employee registered was under maternity
leave;

3. This affidavit is being executed to attest to the truth of the

foregoing facts and for whatever legal purposes this may deem
serve;

4. Affiant further sayeth naught.
