AFFIDAVIT OF TERMINATION OF EMPLOYMENT

I, CELESTINA R. MANAUG, of legal age, Filipino, and with residence at Reyes

St., Mabuhay, Carmona, Cavite, after being sworn in accordance with law, depose and
state:

1. I am a bona fide member of Social Security System (SSS) with assigned

SSS No. 03-8110903-1;

2. I am a former employee of Reliance Producers Cooperative with office

address at 7 Golden Mile Avenue, Golden Mile Business Park, Brgy. Maduya,
Carmona, Cavite from 16 April 2018 to 27 January 2021;

3. I was terminated from employment effective 27 January 2021 due to

retrenchment;

4. I cannot secure a copy of a notice of termination from my employer; and

5. I am executing this affidavit to attest to the truth and veracity of the

foregoing statements and to support my application for unemployment insurance or
involuntary separation benefit.
