Description of the incident:

I noticed one student kept on pacing around and had difficulty in staying in one place. He
became also more emotional and sometimes acted out towards his peers.

Description of the location:

The incident happened inside the classroom, the weather was fine and there’s enough air
passing through from the outside.

Notes/Suggestions/Actions:

I asked the student what causes his behavior and he responded that he was experiencing
frustration particularly in following the steps in his accounting subject. I talked to him, told him to
acknowledge his feeling, do exercises to release his tensions and reminded him that it’s normal
and temporary. As their teacher in accounting, I suggested alternative ways in finishing his tasks
and also helped him from then on build up his confidence.

I noticed

The incident happened inside the classroom. Our classroom is well-ventilated and the
surrounding was generally calm.

Notes/Suggestions/Actions:

To accommodate visually impaired students in the class, I do day-to-day actions to help

them succeed. When writing on the board, I always verbalize what I am writing so that they have
access to that information and can follow along. I established correct seating, planned movement
routines and transitions, and became open to listening constructive feedback and adjusting my
methods and practice on the fly a bit as stuff comes up. Also, I talked with the parents thru our chat
group and home visitations about this learning situation of their children.

Description of the incident:

I noticed one student experiencing a severe stomachache.

Description of the location:

The incident happened inside the classroom, the weather was fine and there’s enough air
passing through from the outside.

Notes/Suggestions/Actions:

Upon learning the situation, I immediately advised the student to visit the rural health center
and instructed some of his classmates to accompany him. I wasn’t able to go along with him
because I have also a leg injury and I cannot still walk normally. I also informed right away his
parents. When he consulted with a doctor in a hospital a day after, I was informed that he was
diagnosed with appendicitis. When he came back to school, I reminded him not to limit physical
activity and strictly follow the advises and recommendations of his physician.
