AFFIDAVIT OF SELF- ADJUDICATION

I, Ma. CRISTINA VALDEZ, Filipino, of legal age, single, and a resident of Brgy.
Luyahan, Lian, Batangas, after having been duly sworn to in accordance with law, do
hereby depose and state, THAT:

1. I am the heir of the late LOLITA VALDEZ, widow, being her biological daughter;

2. The late LOLITA VALDEZ passed away on December 29, 2019 and died without any last
will and testament;

3. At the time of death my mother had no debts, liabilities or obligations to any persons,
agency or institution;

4. That my late mother is the co-owner of a parcel of land covered by Transfer of Certificate of
Title No. 055-2015000902 situated in the Municipality of Lian, Province of Batangas, which is
more particularly described as follows:

TRANSFER CERTIFICATE NO. 055-2015000902

5. As the heir of the herein decedent, I hereby adjudicate her share on the abovementioned
property solely for and in my name as well as any and all liabilities arising from the said
property;

6. I hereby execute this affidavit for the purpose of settlement of the estate of the late LOLITA
VALDEZ, as well as for the sale of her share to any third person natural or juridical.
