Dear School Head,

The Department of Education (DepEd) is committed to ensure teaching and learning continuity amidst the COVID-19 pandemic, while
looking after the health, safety and well-being of its learners, teachers, and personnel. With the sudden shift to alternative learning
modalities, it becomes more crucial for schools to adapt assessment and grading practices that meaningfully support learner
development in varied contexts, and thereby certify achievement at the end of every quarter using grades.

To contribute to a deep understanding of how our learners are doing as indicated by their grades in the First Quarter of this School
Year, please provide all the necessary information accurately in this online form. Results will be reported in groups and will be used
only for research purposes and for informing policy-related decisions and actions. Rest assured that all data shall be kept strictly
confidential and in compliance with Republic Act 10173 or the Data Privacy Act of 2012.
