FAQs – About the forms in SIMS Content Viewer


Q: How do I find a form?
A: Go to Catalog and search for the form part number using the legacy Adobe DOC# or the HTML form
number (formerly known as a SIS).

Alternatively, you can use the Catalog filters - modality and product system, language, and status - then
click [Search] button. After the results list is displayed click on the heading Maintenance & Regulatory
Forms. Use the keyword Search field to narrow down results further.

Another option is to use the Subscription feature to subscribe to all forms for a selected modality and
product system.

Q: Will forms automatically download in SIMS Content Viewer?


A: Forms will auto-download if a subscription is created in the My Subscriptions screen for Maintenance
& Regulatory Forms document type. Otherwise, the forms will not automatically download into My
Procedures.

However, newly released revisions to forms previously downloaded to My Procedures will auto-
download. You can find the forms by typing the legacy Adobe Doc # or the HTML # in the Catalog
Search. The search will return the form.

Q: What if I followed the directions but unable to find the form?


A: For a few forms that go across multiple modalities please click the Browse By Products button and
select the modality General Service Forms. This would include FDA 2579 Assembler Form, Generic PM
Form, and the Calibration Record and Tube Change Form.

Q: If there is no new HTML version of the form can I still use the Adobe version?
A: Yes, once the Adobe version is filled out there are two options to submit to the new repository
(https://sims-forms.cloud.gehealthcare.com/#!/main)

Note: The Adobe PDF forms obsoleted in 2022 have HTML (SIS) versions available.

• PM or Install forms are uploaded through the Service Forms Repository using the Upload Adobe
form tab. Select the type of form in the Form Type drop-down menu, select the form to upload
from your local machine, and click OK. The system will immediately validate if the form was
submitted successfully and will notify the user.

For other special forms like Out of Tolerance or regional specific forms that have not been converted to
HTML, please attach the completed Adobe form to the HTML Generic Form (BBDCN0966) and submit
through SIMS. This can also be an option for any Adobe form that will not upload into the repository
using the Upload Adobe Forms tab mentioned above.

Q: What is the new Part Number (SIS ID) for the HTML General Service Form for Tube
Change / Cal Record Form?
A: The Part Number SIS ID is BBDFF0966.

Q: What is the new Part Number (SIS ID) for the Generic HTML form?
A: Generic HTML (SIS) Form Part# BBDCN0966

Q: Why are the Results section different than the Adobe version of the form?
A: To standardize the form templates, a standard drop down was implemented for most tasks for all
converted forms.

Q: Are special characters accepted in the HTML form?


A: Yes, special characters are allowed in the HTML form.

Q: Can I share/split a partially completed HTML form with another FE?


A: At this time SIMS does not have a mechanism to allow an FE to split an incomplete form. Only
complete forms can be submitted to the repository.

Q: Can I attach a form within an HTML form?


A: Forms cannot be attached. Each form needs to be completed and submitted individually to the
Digital Service Form repository.

Q: Will the new HTML form attach to Siebel like the PDF forms did?
A: Yes, the same solution that worked with the Adobe forms has been implemented with the HTML
version of the Digital Service Forms. If there is a case where the new form does not attach to Siebel,
the form can manually be attached to the SR.

NOTE: Install Digital Service Forms will not automatically attach to the SR. Users must manually
attach the Install form to the SR. PM, CR, and FMI Digital Service Forms will automatically attach to
the SR.

Q: Who should I contact if I need to void a form?


Introduced in SIMS 9.3 / Content Viewer 2.1.04
A: Please contact your Regional PM Operations Team or QA Team to void any forms.

Q: How do I know what fields are mandatory to submit the HTML form?
A: Scroll down to the bottom of the form and click on the Validate Form Button. It will highlight all
mandatory fields in red.
NOTE: If a task is Fails, is Not Complete, or N/A, the Comments field is mandatory.

Q: Is it possible to copy or duplicate an HTML form that has been filled out?
Introduced in SIMS Rel. 9.4 / Content Viewer 2.1.05

A: In Saved Forms view click the icon that looks like two sheets of paper under the Action column.

Once clicking on the icon, a pop up will ask if you want to copy a version of that form.

If Copy Content is selected a copy of the form is found at the bottom of the Saved Forms list with a blue
icon with a “C” to note this is a copied form.

NOTE: Not all fields are able to be copied. Global Services QA confirmed the fields that could be copied.
These fields are in the top of the form and in the bottom of the form. No task results or task data can be
copied into another form.

Q: Can I have multiple HTML forms open at the same time?


Introduced in SIMS Rel. 9.4 / Content Viewer 2.1.05

A: Yes, with the new Tab functionality that was released this can be done. You will be able to open and
toggle between up to 8 forms at one time with this new functionality. The tabs will show up at top of
My Procedures page for the open forms.

Q: Where can I find HTML forms I’ve filled out and saved before submission?
A: On the My Procedures page in SIMS Content Viewer there is a View drop-down menu. Select the
option Saved Forms. Any forms saved and not submitted to date will display on this page.
NOTE: Saved forms will be removed after a laptop swap or if user upgrades Content Viewer to a
different location on their hard drive from the previous installed location.

Q: When I fill out and save a form, can I save it on my local machine?
A: An HTML form is saved in SIMS Content Viewer before being submitted and cannot be saved on a
local machine. Once an HTML form is submitted, a PDF version of the form will be included in the
confirmation email (you must be using SIMS Content Viewer version 2.1.00 or newer). This copy of the
form can be saved or emailed to a customer.

Q: Is there a way to print the HTML form before submitting to the repository?
A: HTML forms can be printed before submission by clicking the printer icon on the upper left corner of
the form.

NOTE: Forms printed before submission may have some text or data cut off. This is due to the form
being printed from Windows. Submitted forms will not have any text or data cut off.

Pre-submitted forms are not intended for customers. Please provide the customer the submitted form
auto-generated PDF copy delivered via email or downloaded from the repository.

Q: Is there any other way to save data in the HTML form besides clicking the Save Button
at the bottom of the form?
Introduced in SIMS Rel 9.2 / Content Viewer 2.1.03

A: Auto-save functionality is available in release 2.1.03 and newer and will save data that is entered into
the form every 15 seconds.

Q: Why doesn’t the HMTL form have a Form Complete checkbox?


Introduced in SIMS Rel 9.2 / Content Viewer 2.1.03

A: Going forward the Form Complete checkbox are being removed from the new Digital Service Forms.
It is not needed because the form is completed within SIMS Content Viewer and then submitted to
repository. No changes can be made to the form so the locking functionality of the Form Complete
checkbox is no longer needed.

Q: Why does Form Status show Incomplete after all fields are filled in?
Introduced in SIMS Rel 9.2 / Content Viewer 2.1.03

A: Users can ignore this radio button status on the form because the DSF repository will show it as
Complete after uploading with a real-time validation feature. Only completed forms will be accepted by
the DSF repository.
Also, the Form Complete checkbox on the forms is no longer needed. As the form is being filled out a
real time validation is being done. Forms missing required data will not be accepted by the repository (a
form cannot be submitted unless it is complete). Note that the Form Complete checkbox will be
removed from all new forms, and as updates are made to the legacy forms the Form Complete checkbox
will be removed at the time of the update.

Q: How do I know if I have saved forms that are expiring?


Introduced in SIMS Rel. 9.4 / Content Viewer 2.1.05, updated in SIMS Rel 10

A: The My Procedures screen will show Forms Expiring at the top-right of the content viewer screen with
an orange circle and exclamation point. This is an alert that a form currently in Saved Forms has a new
revision, and the user will have a total of 10 days to complete submission of that saved form before it is
deleted from the Saved Forms list.

The My Forms screen displays a red triangle on the row of any form that is expiring, and the Expires In #
of Days column shows number of days before the form will be deleted.

Q: If an HTML form is voided to make corrections, and need to resubmit it, is there any
way to update just the item that needs to be changed?
Updated in SIMS Rel. 9.9 / Content Viewer 2.1.11, re-introduced in SIMS Rel 9.13 / Content Viewer
2.1.15

A: The Reload Submitted Form Data button at the top right of the form template can be used to reload
data on HTML forms.

NOTE: Submitted HTML form IDs are only saved for 14 days. Users must
acknowledge they are not using this Reload Submitted Form Data functionality to
pre-populate a new PM/CR/Install form.

Once the Reload Submitted Data button is clicked all submitted HTML form IDs for that form are listed.
Select the Form ID for the form that was voided, and that completed form will be displayed.

Make the needed updates to the form that was voided and submit the updated form with the
corrections.

Q: Do I need to be online to submit a new HTML Form?


Introduced in SIMS Rel 9.1 / Content Viewer 2.1.02

A: You no longer need to be online to submit an HTML form. While offline a user can click the Submit
button and will get a pop-up message stating the form will automatically submit the next time user logs
into the Content Viewer (online). For any form submitted offline the user will see the two pop up
messages below:

The user can find the HTML forms submitted offline by selecting the Offline Submitted option in the
View menu drop down on the My Procedures page. When offline those forms will show up in as
Pending.

Once back online and logged into the SIMS Content Viewer the status will move to In Queue.

Once a form is successfully submitted it will be removed from the Offline Submitted list. The user will
receive the confirmation email like they would for an online submitted form.

If for any reason the form fails to be successfully submitted to the repository, please created an incident
with the SIMS Support Team so they can help with this issue and ensure the form submits successfully.

FAQs – About the submitted forms in Service Forms Repository

Q: How do I request access to the Service Forms Repository?


A: Click here for the Service Forms Repository request form, and select Forms User in the request.

Q: How do I request support for the Service Forms Repository?


A: Click here for the Service Forms Repository Incident form. Provide the details of the issue and click
Submit button.

Q: Where can I find my submitted form?


A: Submitted forms are in the Service Forms Repository.

Q: Why do I have issues opening some forms in the repository?


A: All converted forms now have a PDF version of the form so the radio button at the top of the
repository will default to the PDF, and a PDF version of the form will be opened when the Form ID is
selected. This will work for legacy Adobe forms and new HTML forms. If for some reason the radio
button was changed to HTML, and this was for a legacy Adobe form a message will pop up saying the
PDF forms will not have an HTML version. Change the radio button to PDF.

Q: How do I upload an Adobe 2579 form to the repository?


A: The PDF version of the FDA 2579 form cannot be uploaded to the new repository because of the FDA
Acknowledgements. You need to use the new HTML FDA 2579 form and submit from the SIMS Content
Viewer. It is Part # BBKMP1169.

Q: Why am I not getting ACK3 and Accession Number from the FDA for the FDA 2579
form (US only)? UPDATED JUNE 2022
A: The FDA shut down the Gateway on May 25th, 2021 and will not be restored. The new process as of
June 3rd, 2022 is the Service Forms Repository generates a unique ID for each FDA 2579 form submitted.
The form is processed and sent to the customer and the state.

Q: What if the legacy Adobe form does not successfully upload into the new repository?
A: We have learned that some authors of forms did not use the standard template and this issue is
happening. For those forms the workaround is to attach the legacy Adobe PM and Installation form to
the Generic HTML form Part# BBDCN0966.

Q: How long will it take to receive the confirmation email with a PDF copy of the
completed form?
A: The validation and submission of completed form to the repository is immediate. An email should be
expected about 2 to 3 minutes after a successful submission. It should not take longer than 5 minutes.

Q: How can I get a PDF copy of a submitted form to save/share?


Introduced in SIMS Rel 9.0 / Content Viewer 2.1.00

A: When a form is successfully submitted a PDF version of completed form will be attached to the
confirmation email. If you did not receive the email with PDF attachment you may download a copy
from the Service Forms Repository. The format in the repository defaults to the PDF version of the
completed form, however, there is also an option for an HTML version of the form if it is needed.

Q: How do I open attachments in the PDF version of the form?


Introduced in SIMS Rel 9.2 / Content Viewer 2.1.03

A: Attachments in a PDF version of the form can now be opened.

To open the attachments (in Acrobat):

• Click the vertical gray bar along the left edge of the PDF to open thumbnail pane
• Click the paperclip icon in the thumbnail pane

• The Attachments list will now display; double-click on the attachment to open

To open the attachments (in Nitro):

• Click the vertical gray bar along the left edge of the PDF to open thumbnail pane

Q: If I want an HTML version of the form how do I get a copy and share?
A: Select HTML radio button at top of list of forms. When the zip file is displayed at bottom left corner
of repository page select Keep and the page will open to Extract the form. Select Extract All as seen
below in the screen shot. Next select the destination to store the extracted form on your local machine.
You can then find a version of the extracted form on your local machine. See Screen Shot below.

Q: What does the Availability column represent?


A: The Availability column is shown in the Submitted Forms screen and FDA 2579 Forms screen and
represents the presence of the PM form in the repository as a PDF or HTML.

If Availability column shows FALSE, it means both the HTML and PDF are not available.

Q: Can I download the search results of multiple submitted forms?


A: On the Submitted Forms and FDA 2579 Forms screens click the EXPORT button to download an Excel
report of the search results of submitted forms.

Q: How do I download multiple submitted forms as PDFs at the same time?


A: On the Submitted Forms and FDA 2579 Forms screens there is a DOWNLOAD button that can be used
to download up to 25 selected forms as PDFs.
Select the desired PDFs by checking the box on the left side of the submitted form then click the
DOWNLOAD button. A .zip file with the selected forms will be downloaded.

Key Changes in this update:

• Digital Service Forms: Reload Submitted Form Data functionality restored for all forms to
allow users to resubmit without having to complete an entirely new form.
o Allows the user to make a small change or correct an issue without having to complete a
totally new form.
o A button is available on the form, on clicking of which old data will populate when user
is resubmitting a form.
o User needs to re-submit form within 14 days of first submit, after which system will not
populate data on button click.
o Requires user acknowledgement of not using the Reload Submitted Form Data
functionality to pre-populate a new PM/CR/Install form.

Key Changes in this update:

• Digital Service Forms repository has changed ownership to a new team, and now has a new
name, Service Forms Repository. Search with the new name in IDM to request an account, and in
My tech / SOS to request support.
• Majority of legacy Adobe PDF service forms have been retired and have been converted to
HTML (SIS Type = Digital Service Form). Please refer to the list of forms that will remain Adobe
for those still in use.

SIMS 9.5 / Content Viewer 2.1.06 / 10-DEC-2021


Key changes in this update:

• SIMS Content Viewer: Users can now search for documents using PSI code.

• Digital Service Forms: Revision and SIS ID are displayed at the bottom of PM form template.
o This information will be displayed when viewing or printing the form.

• SIMS Content Viewer: A warning message alerts users when trying to open more than 8 SIS
forms or documents at once.

• Digital Service Forms: Forms retain entered date after opening a saved form.
o (Defect fixed) The date use to revert to the previous day’s date.

Key changes in this update:

• Content Viewer: TAB functionality within CV for HTML (SIS) Forms and documents.
o Allows user to have multiple SIS documents open at the same time with each one in its
own ‘tab’ in Content Viewer (browser-like strategy).

• Content Viewer: Inclusive search results within a document classification.


o Ensures users retrieve all documents in a search even if there are sub-classifications.

• Digital Service Forms: Copy & Save As Functionality in HTML Forms (SIS).
o Allows users to pre-fill out multiple forms’ header (customer info) and footer
(comments, tools, and attachments section) reducing the need to re-enter same details
on each form.

• Digital Service Forms: Open multiple instances of same HTML (SIS) form.
o Allows the users to open & fill out the same form multiple times at the same time.

• Digital Service Forms: Notify user when a saved HTML (SIS) form is about to be removed.
o This feature provides a visible notification symbol (red triangle) on the Content Viewer
header, next to My Procedures in the menu, and besides each saved form that will

• Digital Service Forms: Ability to open / fill out multiple forms at the same time – if they are

o Allows users to open multiple forms at same time and fill out.
o The ability to fill out multiple instances of the same form will be available in the next

• Digital Service Forms: Update a voided form without having to complete an entirely new form.
o Allows the user to make small change /fix issue without having to complete a totally
new form.
o A button will be available on form, on clicking of which old data should populate when
FE is resubmitting a Form.

o FE needs to re-submit form within 30 days of first submit, after which system will not
populate data on button click.

• Digital Service Forms: Keep saved version of form to prevent data loss.
o Prevents filled out/saved forms from being removed from a user’s Content Viewer when
a new form is published, or deprecated.
o Saved form is retained for 10 days.

• Digital Service Forms: Voided forms will show as Incomplete in repository.


o This allows the user to re-upload / re-submit the corrected form.

• Service Forms Repository: Ability to search for multiple serial numbers, System Ids, and
Service Records in repository.
o Primarily for LCS forms that may contain multiple S/N, system Ids, and/or SR#.
o Only applies to PM forms.
o Search is flexible enough to find form if a Serial Number, Service Record, or Sys ID is not
the first one listed in the form.

• Digital Service Forms: Offline submitted forms are not removed when new revision published.
• Form in offline submitted state will be submitted to back-end for below to scenarios.
