agree to become a BRIGHT MINDS tutor who is committed and I will comply with the conditions that have been set below:

1. Hand in a copy of the teaching and learning (T & L) for each student which has been signed by the client no later than the 2nd of the following month to the
BRIGHT MINDS manager.
2. Email any receipts to BRIGHT MINDS at the end of the month for claims of additional exercise books (tutors are allowed to claim a maximum of RM30/year for
each student).
3. Email claim information to brightminds@gmail.com together with a copy of the teaching and learning report (T & L) to the manager. This will be approved
by the 30th, and allowances will be banked in by the 7th of the following month.
4. P & P is only for 4 weeks a month. Week 5 is only for replacement classes, meetings, trainings and other purposes. Any changes to the schedule must be
informed to the BRIGHT MINDS management and clients.
5. A copy of T & L will be given to the clients at the end of every month.
6. It is compulsory for me to attend each meeting and training programmes.
7. I also understand that failure to do so will cause my allowances to be deferred.
8. I will ensure student receive the starter kit that has been prepared by BRIGHT MINDS management.

If I am a new tutor, I will be committed and abide by the rules and guidelines that have been set by the management. I WILL NOT ASK about any payment rate that
has been set by the management with the client and I WILL NOT TAKEOVER any current clients for my own personal teaching services. If found guilty, I will be
charged in court and I will pay for every expenses incurred in court.

If I want to stop teaching, a one-month notice in the form of letter will be given. Failure to give notice will
result in no teaching allowance paid to tutor.

I will also maintain a pleasant and respectful personality as a tutor in terms of attitudes, manners and attire, upholding my good name, religion & of BRIGHT MINDS
HOME TUITION. I will also be punctual and to teach according to a predetermined time with clients and BRIGHT MINDS HOME TUITION. I may be terminated at any
time if I violate the rules or as requested by the client.

I also understand that if I stop teaching a client, I will not continue teaching the client for at least within a year.

I agree to open an account with CIMB Islamic to accelerate the process of payment of allowances from BRIGHT MINDS HOME TUITION. I also agree to be a member
of BRIGHT MINDS HOME TUITION (Closed Group) on Whatsapp to keep up with the latest information by BRIGHT MINDS HOME TUITION from time to time. Thank
