Inserting an IV Cannula Checklist

Skill Performed
Introduction
Wash hands using hand gel or soap.
Introduce self and Explains role.
Gather basic patient details (name/DOB).
Explain the need to insert IV cannula and gain consent.
Ensure the patient is comfortable & privacy.
Address confidentiality.
Preparation
 Ensure that all appropriate equipment is readily available.
o Clean procedure tray
o Non-sterile gloves
o Tourniquet
o Cannula (size appropriate to the indication for cannulation)
o Cotton balls
o Cannula dressing
o Luer lock cannula cap or extension set
o Gauze swabs
o Normal saline 0.9% (10 ml)
o Syringe (10ml)
o Include sodium chloride flush (double checked) and primed extension line, if
required.
o Alcohol swab
o Sharps container
 Wash hands and put on nonsterile gloves
Patient position
 Position and expose the arm and identify a suitable vein.
 Apply tourniquet and recheck vein. Loosen until just before procedure.
 Ensure patient is ready to proceed and explain what they will feel.

 Clean target area and allow to dry.

Cannula preparation
 Remove the cannula from its pack using a ‘non touch’ technique.
Cannula insertion
 Re-apply tourniquet.
 Stretch the skin and insert the needle, bevel upwards, in line with the vein at approx. 30
degrees.
Cannula advancement
 Advance needle until blood flashes back into hub then advance the cannula without the
needle.
Tourniquet release
 Release tourniquet when second flashback.
Needle removal
 Withdraw needle and discard it in the sharps bin.

Cannula extension
 If required, whilst supporting cannula, screw cannula extension into place (pre-primed).

Cannula management
 Secure cannula using tape and / or appropriate dressing. Flush cannula with saline.

To complete the procedure


 Dispose of sharps and clinical waste in appropriate waste bins.
 Remove gloves and wash hands and dispose of PPE
 Explain to the patient that the procedure is now finished.
 Thank the patient for their time.
