EMPLOYEE PERSONAL DATA ACKNOWLEDGEMENT/CONSENT

“Acuity” means Acuity Knowledge Services (India) Private Limited or one of its affiliates within the
Acuity Knowledge Partners business unit, employing or proposing to employ you on the date you sign
this document.

In order to maintain consistent practices and procedures with respect to the collection, use, disclosure,
transfer and processing of employee data by all Acuity affiliates worldwide, Acuity collects personal
data relating to you for the purposes of your employment and/or compliance with applicable laws,
rules, regulations, policies and procedures. These include, but are not limited to, the effective
recording, management and administration of human resources data and performance of the human
resources functions of Acuity, collecting and processing securities accounts, holdings and transaction
information provided by you, processing and providing you with compensation and benefits information
and services and evaluation of your individual performance. Acuity may forward some of these data to
other members within Acuity for storage and processing, for complying with the functions that each of
them may perform relating to regional or global HR decisions within Acuity, for benchmarking Acuity
employee salary and benefits with similar organizations, for assessing compliance with internal
policies, or for providing the employees of Acuity with information for the performance of their jobs.
Acuity may also use agents or contractors, bound by obligations of confidentiality, in connection with
performing the foregoing functions, including but not limited to, outsourced payroll or HR service
providers, IT and communications service providers, law firms, accountants and auditors.

These data are initially collected through the recruitment process and the completion of a form by each
employee at the time he or she joins Acuity including full name, date of birth, nationality, education and
qualification details, marital status, home address and home telephone number, mobile telephone
number, number of dependents, next of kin details (e.g. spouse, parents, children, brother/sister
names, home addresses, telephone numbers), bank account details for transfer of salary and other
benefits, tax details and date of hire. Other data are collected in the course of employment and include
data on an employee’s work related details such as position, contact details, performance evaluation,
absences, pay and benefits, service history, copy of employment agreement, passport details,
photograph, health information, pregnancy and/or disability status etc. Personal data may also be
collected through your use of our email system for compliance purposes through the email monitoring
process which has been implemented in conformity with applicable laws. This includes but is not
limited to your full name, email address and the content as well as date and time of your email
correspondence. Private contact details relating to you and next of kin that you have provided to Acuity
for emergency purposes shall be used by Acuity only in connection with an emergency. Acuity also
collects information from relevant employees relating to his or her securities accounts, holdings and
transactions, as well as the securities accounts, holdings and transactions of certain of his or her
family members, in order to ensure compliance with Acuity’s internal policies. In addition, Acuity may
collect other information through exit interviews from departing employees (including future employer
and position details) in order to comply with applicable laws, to analyze the employees’ reasons for
leaving, and, as the case may be, to maintain contact with those employees in the future.

Your personal data are entered in centralized databases hosted and maintained by Acuity including in
jurisdictions as applicable in furtherance of Acuity’s bonafide business purposes. The majority of data
in these Acuity databases are accessible only by certain personnel of Acuity for the purposes of
management and administration of the databases. Some data also may be accessed where necessary
by Acuity personnel for the purposes of assessing compliance with applicable laws, rules, regulations,
policies and procedures. Some limited data may also be accessed by Acuity senior management for
the purposes of HR management and administration. Where the data are held as part of an internal
directory (such as the Employee/Associate Directory or the HR Self-Service Center), that data may be
accessed by certain other employees of Acuity for ensuring an adequate communication with you for
the performance of the relevant employment duties or for emergency reasons. Further, Acuity may
release your data to third parties if required by law, regulation, or court order.

Your personal data can be collected, used, disclosed, transferred and/or processed as set out herein
during the continuance of your employment with Acuity and thereafter, as long as reasonably
necessary for Acuity’s legitimate business purposes and as permitted by applicable law.

The purpose of this letter is to provide you with the relevant information on the collection, storage, use,
disclosure, transfer and processing of your personal data controlled by Acuity and to seek your
consent thereto, including without limitation, the transfer of your personal data for the purposes and to
the transferees described above.

Please acknowledge your consent to the collection, use, storage, disclosure, transfer and processing
of your personal data by Acuity as described above by signing and returning this form to your
appropriate HR contact by or on the first day of your employment with Acuity. While you are not
required or obliged to provide your consent, as an employee of a multinational corporate group, we
request that you do so to ensure that your data can be maintained in a consistent, effective, and
efficient manner.

EMPLOYEE’S CONSENT

I hereby confirm that, with effect from the date when I first provided any personal data to Acuity:

(a) I understand the terms and conditions of the collection, use, disclosure, storage, processing and
transfer of my personal data by Acuity and I hereby consent to such collection, use, disclosure,
storage, processing and transfer of my personal data as described above;

(b) I hereby further consent to the transfer of my personal data by Acuity to transferees in jurisdictions
as applicable in furtherance of Acuity’s bonafide business purposes;

(c) I am aware that my personal data in Acuity database will be accessible to other Acuity companies
in different countries; and

(d) I confirm that I have the consent of any relevant next of kin and family members for the use of their
personal data in accordance with the purposes and in a manner described above.
