AFFIDAVIT OF INCOME

I, GEMMA RAZON SARMIENTO, Filipino, of legal age and resident of B5

L2 Istana Subd. Phase C, Malagasang 1-F, Imus City, Cavite, after being duly
sworn to in accordance with law, depose and state: That:

1. I am applying for the membership re-structuring housing loan with Home

Development Mutual Fund also known as Pag-ibig;

2. As such I was required to declare my monthly income;

3. I am earning Fifteen Thousand Five Hundred Nine Pesos (Php15, 509.00)

a month which derived from my work as Operation Assistant;

4. And also, I am earning an additional income of Twelve Thousand Pesos

(Php12, 000.00) a month which derived from my Sari-Sari Store with a
total monthly income of Twenty-Seven Thousand Pesos (Php 27, 000.00)’

5. Through this Affidavit I hereby declare under oath that I am earning a total
monthly income of Twenty-Seven Thousand Pesos (Php 27, 000.00)’,
which I derived from my above-mentioned work and business;

6. I am executing this Affidavit to attest to the truth and veracity of the

foregoing and for whatever legal purpose the same may serve.
