AFFIDAVIT OF SINGELNESS

I, CARMEN T. AMACIO, legal age, Filipino, single and presently residing

at # 38 Regatta Classics Subd., Anabu II-B, Imus City, after having been duly
sworn to in accordance with law, depose and state that:

1. That the person and individual of CONSEJO PRADO AMACIO was

personally and familiarly known to me for simple reason that the subject
is of legal age, having been born on April 25, 1939, in Malay, Aklan;

2. That CONSEJO PRADO AMACIO died on August 10, 2021 in

Tanza, Cavite;

3. That CONSEJO PRADO AMACIO has no child or children;

4. That I am executing this affidavit to attest to the truth that CONSEJO

PRADO AMACIO was single until the last day of her life, and has
never been married before, in compliance with the requirement and for
any other legal purpose this may serve.
