Alfonso Village, Alapan 1B, Imus City, Cavite, Philippines, having
been duly sworn in accordance with law, hereby depose and state:

1. That I am the daughter of PRICILLA P. ORDILLO;

2. That she is the owner of the following certificate:
a) CERTIFICATION issued by City Government of Imus
Office of the Punong Barangay on June 9, 2022,
signed by Brgy. Capt. ROLANDO MARCIANO;
3. That the above-mentioned Certificate was issued in the name
of my mother;
