NEW NORMAL HEALTH ASSESSMENT FORM FOR LEARNERS.

1. Health assessment plays a critical role in evaluating and determining a learner's
health status. This will identify the specific needs of the person and how those needs will
be addressed,
2. __ In line with this and in lieu of physical examination, each school is advised to adopt
the New Normal Health Assessment Form for Learners. This will help detect the early
signs of certain health conditions and address them accordingly. Likewise, this will aid in
the profiling of the learners and serve as basis for future stakeholders’ support,
Furthermore, this will also augment in the implementation of the Oplan sa Kalusugan sa
DepEd in these challenging times.
3. . Funds for the printing of this form shall be chargeable against the school
MOOB/local funds subject to the usual accounting and auditing rules and regulations
4, Attached herein is a copy of the New Normal Health Assessment Form for Learners
5. Immediate dissemination of this Memorandum is desired.
