AFFIDAVIT OF WAIVER

I, CAROLJEAN COSMILLE O. AGGABAO, of legal age, single, Filipino, and with

residence address at San Fermin, Cauayan City, Isabela and with office address at PROVIDERS
MULTI-PURPOSE COOPERATIVE, 1AH26 Magsaysay, Naguilian, Isabela, after sworn to
according to law, hereby depose and state that:

1. That I am the Acting Chief Executive Officer of PROVIDERS MULTI-PURPOSE

COOPERATIVE (PMPC);

2. That I am the authorized representative of PROVIDERS MULTI-PURPOSE

COOPERATIVE (PMPC);

3. The PROVIDERS MULTI-PURPOSE COOPERATIVE (PMPC) is named as one of the

lawful owners of the motor vehicles described in the list of OR/CR for cancelation of
PMPC Encumbrance, copy of which is attached hereto as annex “A”.;

4. In behalf of PROVIDERS MULTI-PURPOSE COOPERATIVE (PMPC), I am waiving

all rights, interest and participation over the said motor vehicles in favor of those
particularly name in each of the motor vehicles provided in the same document attached
hereto;

5. That this waiver does not in any way bar other interest that PROVIDERS MULTI-
PURPOSE COOPERATIVE (PMPC) may have arising from the other contracts entered
into which the motor vehicle is subjected to;

6. That I am executing this affidavit to attest the veracity of the foregoing facts and for all
the intents and for whatever legal purpose it may serve in the process of transferring the
registration of the motor vehicle under the name of the aforementioned person.
