• Medical History. Provide the

necessary information on the previous
illnesses experienced in the last 6-12
months, immunization / vaccines
received, medicines that you are
currently taking, allergies, disabilities,
previous hospitalizations, surgery
undertaken, accidents and other
injuries, illnesses that your family
(history) have, and the onset of your
puberty (pubertal history).

Flow of Medical and Dental Examination:
A. For returnees or enrollees with existing Student Health Record (SHR) in
the Health Services*:
o Enrollees / returnees should verify their record at the Campus Infirmary.
o Secure an order of payment from the Health Services and pay the amount of Php
50.00 for Medical Certification Fee at the Cashier’s Office.
o Update your record by following the instructions of Health Services personnel.
o All the submitted requirements will be evaluated by the authorized personnel.
o A medical certificate (addressing to the Registrar) will be released to the enrollees if
they meet the requirements needed.
*Student Health Records from 2015-2021 are available in the Health Services (students with ID Number starting from 15-, 16-, 17-, 18-, 19-, 20-, 21-, 22-)

B. For new enrollees:
o Secure an order of payment from the Health Services and pay the amount of Php
50.00 for Medical Certification Fee at the Cashier’s Office.
o Download and print the Student Health Record (SHR) on a legal size bond paper.
Accomplish the SHR by providing necessary information and checking the box.
Write legibly.
o Present all needed requirement to the WPU Health Services for evaluation.

Flow of Medical and Dental Examination:

B. For new enrollees:
o All the submitted requirements will be evaluated by the authorized personnel.
o The authorized personnel will validate the information and assess the body mass index (BMI),
vital signs, visual acuity.
o The University Dentist / authorized personnel will assess the general oral health of the enrollee.
o The University Physician / authorized personnel will conduct physical and neurologic
examination.
o The University Physician / authorized personnel will make impression and diagnosis and release
medical certification to enrollees with essentially normal findings. Enrollees present apparent
clinical contraindications will be referred to laboratory testing for confirmation of diagnosis or
medical specialist for further evaluation and management.
