Your signature is used by DUF as a payment request for the specified account number. At
the same time, you declare that you agree with the terms that apply to accounting and
reporting, and that by signing you declare on good faith that the grant will be used in
accordance with the commitment letter.

The contact person is financially responsible to DUF.

You declare that you agree that grants may be required to be fully or partially repaid if:

1. The activity has not been carried out as described in the application, unless
otherwise agreed with DUF.
2. The activity ends with a profit.
3. DUF cannot approve the accounts and the description of the project.

ATTENTION ! We need your signature, so you must print out the document and sign by
hand. You cannot enter your name on the computer.
