Letter dated 01-03-2022 with employee code 00099431 does hereby
solemnly affirm and declare as under:-

1. That my savings account is with ICICI Bank with IFSC code

ICIC0000202 and account number 020201551438.
2. That my name as per the Appointment Letter is Ghanta Anjana
Phani Kumar in HR records and the name as per Bank Account is
G A Phani Kumar.
3. That I would like to confirm that both the names as stated in point
2 above are same and the Company can therefore process the
amounts due to me in the account number mentioned above at
point number 1.
4. I affirm that the Company is released from the liability due towards
me on account of such payment and I am aware that incase the
information furnished above is found to be incorrect, I shall be
liable for prosecution under section 191 of the Indian Penal Code,
which stipulate as under:
