stated that you would like to withdraw your claims filed before the Associated Marine Officers and Seamen's Union of the Philippines (AMOSUP) against your previous company, Leonis Nagivation Company, Inc.
Please be advised that filing of withdrawal of claims before the
AMOSUP may result in the dismissal of claims with prejudice. As a consequence, your claim for your disability benefits against Leonis Navigation Company, Inc. shall be permanently barred.
