    Sportsfest of the Senior High School Department. This will be held at the Theresian School of
    Cavite on January 27, 2024. I permit my child’s participation in this activity because I believe
    that the activities included in this Sportsfest will help my child showcase his/her athletic skills.

    Further, the student whose name appears above is expected to exercise and follow the school
    guidelines that are required for the safety of his/her well-being on the date, time, and place of the
    activity.

    In addition to giving my full consent for my child’s participation in this activity, I do hereby waive
    and hold no harm against the institution.
