A homeowner discovered their right front tire was flat when getting ready to run errands. Upon inspection, nails were found in the driveway where roofing work had recently been done. A why-why analysis identified the likely root cause as nails from roofing work ending up in the driveway, which then punctured the tire and caused it to lose air.
