NEBULIZATION
NEBULIZATION
• Is a the process of
medication administration
via inhalation

• Involves the use of nebulizer


which is a medical device
that converts a liquid
medicine into a mist, vapor
or aerosol, such that it can
be inhaled directly into the
lungs or the air tubes.
Nebulizers are commonly used for
treatment of:

•Cystic Fibrosis
•Asthma
•COPD
•Respiratory diseases or disorders
INDICATION OF NEBULIZATION
• Adds moisture or medications to inspired by
mixing particles of varying sizes with the air.
Aerosolization suspends the maximum number
of water drops or particles of the desired size in
inspired air.

• The moisture added through nebulization


improves clearance of pulmonary secretions
Some of the common medicines used in
nebulizers include:
• Albuterol
• Budesonide
• Ipratropium
• Levalbuterol
TYPES OF NEBULIZER
a) Jet - Compressed air turns the medicine
into a mist (tiny particles of medicine that
float in the air).
b)Mesh - Medicine is passed through a tiny
mesh to create a fine mist.
c) Ultrasonic - High frequency vibrations turn
medicine into a mist.

PROCEDURE
• Position the patient appropriately, allowing optimal ventilation.

• Assess and record breath sounds, respiratory status, pulse rate and
other significant respiratory functions.

• Teach patient the proper way of inhalation:


Slow inhalation through the mouth via the mouthpiece
Short pause after the inspiration
Slow and complete exhalation
Some resting breaths before another deep inhalation
• Prepare equipments at hand

• Check doctor’s orders for the medication, prepare thereafter

• Place the medication in the nebulizer while adding the amount of


saline solution ordered.

• Attach the nebulizer to the compressed gas source

• Attach the connecting tubes and mouthpiece to the nebulizer

• Turn the machine on (notice the mist produced by the nebulizer)


• Offer the nebulizer to the patient, offer assistance until he is
able to perform proper inhalation (if unable to hold the
nebulizer [pediatric/geriatric/special cases], replace the
mouthpiece with mask.

• Continue until medication is consumed

• Reassess patient status from breath sounds, respiratory


status, pulse rate and other significant respiratory functions
needed. Compare and record significant changes and
improvement. Refer if necessary

• Attend to possible side effects and inhalation reactions


Complications
Possible effects and reactions after nebulization therapy are as follows:

• Palpitations
• Tremors
• Tachycardia
• Headache
• Nausea
• Bronchospasms (too much ventilation may result or
exacerbate bronchospasms)
CLEANING OF NEBULIZER
The recommendation for disposable
nebulizers used in the hospital setting is to
rinse the residual medication out with sterile
water, wipe the mouthpiece or mask with an
alcohol pad after each use, and discard the
disposable nebulizer after 24 h.
• As nurses, it is important that we teach the
patients the proper way of doing the therapy to
facilitate effective results and prevent
complications (demonstration is very useful).
Emphasize compliance to therapy and to report
untoward symptoms immediately for apposite
intervention.
Setting Up and Using a Nebulizer
(HOME)
The basic steps to set up and use your nebulizer are as follows:
• Gather your equipments.
• Wash your hands.
• Connect the hose to an air compressor.

• Fill the medicine cup with your medicine. To avoid spills, close the
medicine cup tightly and always hold the mouthpiece straight up and
down.

• Attach the other end of the hose to the mouthpiece and medicine
cup.
• Turn on the nebulizer machine.

• Place the mouthpiece in your mouth. Keep your lips firmly around the
mouthpiece so that all of the medicine goes into your lungs. If using a facemask,
place it over the mouth and nose. Small children usually do better if they wear a
mask.

• Breathe through your mouth until all the medicine is used. This takes 5 to 20
minutes, depending on the device and medicine used. If needed, use a nose clip
so that you breathe only through your mouth.

• Turn off the machine when done.

• Wash the medicine cup and mouthpiece with water and air dry until your next
treatment.
After each use:
• Wash the medicine cup and mouthpiece with warm
running water.
• Let them air dry on clean paper towels.
• Later, hook up the nebulizer and run air through the
machine for 20 seconds to make sure all of the parts
are dry.
• Take apart and store the machine in a covered area
until the next use.
