AFFIDAVIT OF LOSS

I, Nenette Y. Guerrero, of legal age, Filipino, residing at Zamora

Street, Barangay Poblacion,Morong, Bataan, after first being duly sworn
depose and say:

1. That I am the Corporate Secretary of Waste Re-energiser

Philippines Inc., a corporation duly organized and existing under
and by virtue of the Philippine laws with principal office address at
G2 Vista Fino Bldg., Diego Silang St., AFPOVAI Phase 1, Taguig City
1630, NCR (hereinafter referred to as the “Corporation”);

2. That in my capacity as the Corporate Secretary, I had custody and

possession of the Corporation’s stock transfer book;

3. That the above-mentioned stock transfer book was lost/misplaced,

or inadvertently destroyed, and, notwithstanding earnest and
diligent efforts by me and my staff, we have not been able to locate
the same;

4. That I am executing this affidavit for the reconstitution/replacement

of the aforesaid stock transfer book and for any legal intent and
purpose it may serve.
