Sir,

We the MIS would like to recommend a power adapter replacement for Monitor to be used for
Loans Officer Ervin Ligan. We recommend because the existing adapter is not working anymore and has
been found to be defective. The adapter has low voltage and it would not receive power to the monitor
due to damaged capacitor inside of the adapter and we can no longer repair it. The following is the
specification of the said unit.
