3. That the proper handling, labelling and storage of the waste shall be observed;

4. That the personnel involved in the transport shall make available the approved Permit to Transport (PTT)
and manifest form including the emergency and contingency equipment in case of accidents and/or
spills;

5. That the online Hazardous Waste Manifest Form shall be prepared for every transport. The geotagged
photographs of actual loading and unloading of hazardous wastes shall be attached in the online
Hazardous Waste Manifest Form;

6. That all waste shall be treated within forty-five (45) days upon receipt of the TSD Facility as reported in
the online Hazardous Waste Manifest Form;

7. No transport and treatment of hazardous wastes shall be undertaken whenever the Transporter
Registration, TSD Registration and the Memorandum of Agreement between and among the Generator,
Transporter and TSD Facility has expired unless the renewal of which has been approved;

8. That the Permittee, Transporter and Generator shall be jointly and severally liable for any misdeclaration
in all the documents submitted to this Office and for any accident/damage to persons, property and the
environment that may arise on the occasion or in relation to the transport/export of the wastes.

9. Non-compliance to the above stipulations shall be subjected to the penalty provisions as provided under
Section 41 of DAO 1992-29 and Chapter 11 of DAO 2013-22.
