Our Mission

My Summit Adventure’s Preschool mission is to provide a rigorous


educational environment in which the children will learn the method of logic,
thinking skills, and creative play. Students will learn to assess, question, and
look for connections in the environment and in academics. We teach children to
find happiness through achievement. We build a strong foundation that builds
from year to year to help ensure a student’s future success. My Summit
Adventure’s Preschool faculty recognizes and addresses individual learning
styles, enabling them to teach the academic program in dynamic and
meaningful ways. My Summit Adventure’s Preschool is a mix of academics and
play. Play centers are used daily that are embedded with math, language arts
and social interactions.

Our Vision
We strive to uphold an environment that is safe, nurturing, and educational
where preschoolers learn valuable life skills, develop character, make new
friends, and discover new interests, preparing them to become lifelong
learners. We provide up-to-date best practices that encourage hands on
engaging activities where preschoolers learn through their play.

We use The Utah State Board of Education’s Preschool Core Standards,


Accredited Preschool Curriculums, preschool best practices, and hands
on learning approaches to building our curriculum.

About My Summit Adventure’s Preschool

My Summit Adventure’s Preschool is for children ages 3 Years to 5 years old.


Students entering the 3-year-old class must be 3 by September 1st of the enrolling
year. Students entering the 4-year-old class must be 4 by September 1st of the
enrolling year.

At My Summit Adventure’s Preschool, we teach Phonics, Reading, Math, Music, Science,


Geography, Problem Solving, Art, Physical Education and Social Skills. Concepts are
taught using music, art, manipulatives, play based learning centers, and other forms
of self-expression for the purpose of discovery and learning.

Enrollment at My Summit Adventure’s Preschool is on a first come, first served basis.


The date and time of the submitted application and registration fee will be noted to
help us create our class lists. Class sizes are targeted at:
• 3 Year Old Classes: 16 students per class with 2 highly qualified teachers.
• 4 Year Old Classes: 18 students per class with 2 highly qualified teachers.

We will accept applications for current students and siblings in January and will open
enrollment to the public in February.

Attendance Policy

Regular, timely and consistent attendance is critical to the learning process. Families
are encouraged to avoid scheduling vacations, doctor’s appointments, and other such
interruptions during school hours.

We do not offer make up days for absences. If a student misses an extended


amount of time, the student’s spot in the class will only be held for him/her if tuition
is paid and current.

If your child will be absent, please email your child’s teacher.

Visitors

For the safety and security of our students, visitors must check in at the
Elementary Office to receive a “Visitor Badge” to be on campus. To receive this
badge, your name, birthday and Driver’s License number will be required. All adults
must wear ID badges while on campus or they will be escorted from the building
immediately. Teachers and staff will not allow any visitor or volunteer in the
classroom without a “Visitor Badge” from the front office.

Preschool

Please display your visor card given to you so staff members can recognize your car.

We do not offer early drop off times.

If you miss the unloading time, please walk your student inside to the preschool class.

STAFF MEMBERS ARE NOT PERMITTED TO UNBUCKLE YOUR CHILD FROM THE VEHICLE.

***Please do not allow your student to be unattended on the premises at any time.
Please do not allow your student to exit your vehicle without a staff member
present to supervise your student.**

Pick Up

Students may be picked up in the loading area from 11:20AM-11:30AM for AM classes
and 3:10PM-3:20 for PM classes. If a parent arrives after the outlined times,
students may be picked up from the inside of the preschool class. Parents will be
charged $1 per minute after time indicated above.

Parents will be given visor cards at orientation. A visor card will be required to pick up
your student. If you need an additional visor card, please ask your preschoolers
teacher. Otherwise, parents may be required to show identification when picking up a
child.

Display your visor card in your windshield or passenger window so that it is readable
to our staff members. Do not remove the visor card until after all students have
been loaded into the vehicle. Staff members will be present along the
unloading/loading lane. Staff may open the car door to assist preschooler into the
vehicle, but STAFF MEMBERS ARE NOT PERMITTED TO BUCKLE YOUR CHILD INTO THE
VEHICLE.

If you are picking up another student, you must have his/her visor card from his/her
parent in order to do this. Students will not be released to another adult without
written communication to the teachers or without the visor card of the student.

Draper Campus Pick Up/Drop Off Instructions:
● Drop-off is at the curb by the Summit Academy Elementary stairs (By the hill) at
8:20am for the morning classes. For the afternoon (pm) classes drop-off is from
12:20PM to 12:30PM. If you miss us, for your child’s safety, please walk him/her into
the elementary office. Let them know what class your child is in and they will call the
teacher to come get them.
● Loading starts at 8:20am. Please do not arrive early, we do not offer an early drop
off option. Teachers will be outside for drop-off at 8:20am.
● Preschool Pickup is at the stairs by the elementary gym from 11:20am to 11:30am for
the morning classes. For the afternoon classes, it is from 3:10pm-3:20pm by the
elementary gym stairs, as well.
○ If you happen to show up late and miss the pickup times, your student will need to
be picked up from the preschool class. The fee for late pickup is $1.00 per minute.

Bluffdale Campus Pick Up/Drop Off Instructions


● Drop off is at 8:20am for our morning (am) classes, pickup time is at 11:20-11:30pm.
Drop off is at 12:20PM for the afternoon class and pick up is 3:10-3:20PM.
● Drop off and pick up for Bluffdale students will be the school’s main doors. If you
miss us, for your child’s safety please walk him/her into the elementary office. Let
them know that you are in preschool, and they will call the teacher to come get
them.

off option. Teachers will be outside for drop-off at 8:20am for the am class and
12:20Pm for the pm class.

Independence Campus Pick Up/Drop Off Instructions


● Drop off is at 8:20am for our morning (am) classes, Pickup time is at 11:20-11:30pm.
Drop off is at 12:20PM for the afternoon class and pick up is 3:10-3:20PM.
● Drop off and pick up for Independence Preschoolers will be at the preschool door
located at the North West end of the building.
● School starts at 8:20am. Please do not arrive early, we do not offer an early drop
off option. Teachers will be outside for drop-off at 8:20am for the am class and
12:20Pm for the pm class.
● Preschool Pickup is at the preschool door from 11:20am to 11:30am for the morning
classes. For the afternoon classes, it is from 3:10pm-3:20pm by the preschool door, as
well.

• Please choose pants that are easy to get on and off for toileting.

**Leggings, tights, or biker short of any color needs to be worn under skirts, skorts and
jumpers. **

General Information.
Outerwear-Students may wear appropriate jackets, hats, gloves, and boots of their choosing
in the winter months.
Sweaters/Jackets/Sweatshirts-if worn in the classroom or inside the school should be Solid
Navy or Black. Crew Neck Sweaters are allowed, Hoodies are not allowed.
Undershirts-must be white or a solid color that matches the color of the uniform shirt
being worn. T-shirts with logos or lettering are not permitted.
Hats-including hoods, beanies, head scarves or bandanas may NOT be worn in the classroom
or in the school buildings during regular school hours.
Shoes-Students may wear any closed toe shoe or closed heel athletic shoe with a non-
marking sole.
Backpacks-We encourage all students to bring their own backpack to school every day.

Please label all of your child’s items. During the winter we go outside, therefore please
send your child with labeled winter wear. For example, if you send your child with any
of the following item’s coats, hats, mittens (Mittens are best, gloves are difficult and
frustrating for little fingers) please make sure initials are somewhere on the winter
wear. If your child would like to wear snow boots that is fine, but plan on wearing
them the whole time at preschool. It is too difficult to change shoes.

Tuition

Tuition reminders/invoices are emailed the 5th of Every Month.

Tuition is due by the 10th of Each month.

Tuition Paid After the 10th of the month will be assessed a $25.00 Late Fee.

Do not attend Day: 20th of the month; accounts unpaid will result in the
student being unable to attend program until paid in full.

Checks should be payable to My Summit Adventure’s Preschool.


If checks are returned for insufficient funds, the checks automatically go to
collections and the responsible party will be charged applicable fees by the collection
agency in addition to late fees which will be assessed by My Summit Adventure’s
Preschool.

Credit card, debit card and auto withdrawal from checking and savings accounts are
available. If payments are rejected, the responsible party will be charged applicable
fees and late fess which will be assessed by My Summit Adventure’s Preschool.

Failure to pay tuition is an automatic forfeiture of the student’s spot held in the
preschool after the 20th of each month.

Registration Fee

There is a $75 non-refundable application fee for 3 & 4 Year Old Classes due with each
application, due yearly upon registration. This is necessary to hold a spot for your child in the
class of your choosing.

Field Trip Registration Fee


Our 4 Year Old Preschool Classes will go on 3-4 field trips throughout the school year.
There Is a non-refundable $50.00 field trip registration fee due yearly that will cover
the cost of field trips.

Withdrawal

Students can be withdrawn from the preschool with a 30-day written notice to the
Preschool Coordinator. Parents can receive a pro-rated refund for the tuition that was paid
in advance beyond the thirty-day written notice.

Toileting
All preschool students must have independent toileting skills. If your preschooler has
a toileting accident a teacher will call the parents and ask for the parent to either
come and pick up or bring a change of clothes. Preschool Staff are not allowed to
assist preschool students with cleaning or changing of clothes.

Please have your student go to the restroom before school in order to decrease
extra potty breaks. Start your child with a good breakfast each day. Full tummies
help little minds learn.

Monthly Calendar & School Website

Every month our wonderful teachers create their own class calendars to help
showcase what will be taught in your students’ classroom for that given month.
These calendars/ newsletters can be found on our website monthly at:
mysummitadventures.org. Teachers may also send a copy home of the monthly
calendar/newsletter.

upcoming events and information updated monthly. You will also find staff bios and
information about other programs that we offer. Our Facebook page is a great way
to help extended family and friends learn about My Summit Adventure’s Preschool.

Field Trips
During registration, parents are given the option to sign a Permission Form for off
campus trips. Students must have signed permission to attend. A school bus will be
the form of transportation on Field Trip Days; if the destination is within walking
distance, students will receive notice to wear comfortable shoes to walk. Field trips
are a privilege, not a right. My Summit Adventures staff have the right to deny
student participation on off campus trips if a student isn’t adhering to the Discipline
Policy.

Student Expectations
Students in the Preschool Program are expected to be respectful of their peers and
adult group leaders. This includes using appropriate school language, keeping hands and
feet to oneself, following directions when given, and being respectful of the school
building, classrooms, and playground areas. This also includes participation in planned
activities. We expect that students will at least try to participate in all enrichment
activities that their teachers have planned for them.

We also expect them to have as much fun as possible!

Example of Our Daily Schedule

Activity We are learning…

Arrival & Students do the morning routine as they arrive. The routine is
Morning Routine put belongings away, say bye to his/her family and choose an
activity either on the table or floor.

Social Relating Students pick a table or floor activity and do the activity.
Students are learning various literacy, math, science, art, and/or
fine motor concepts/skills. The activities are often connected to
our theme!

Gathering Time We review the calendar and schedule and talk about our day.
There are tons of math embedded into calendar time.

Group Students will work with the teacher in a smaller group to learn
various literacy, math, or science skills and concepts. New games
Math or LA and activities are introduced at this time to ensure student
success. The groups are always changing based on students’
ability, interest, or random selection.

Singing Time Music time is filled with songs, rhymes, instruments, and use of
Music and Movement gross motor equipment. Through music we build and learn
various language skills, phonemic awareness, math concepts,
music, social skills, and gross motor skills.

Group Students will work with the teacher in a smaller group to learn
various literacy, math, or science skills and concepts. New games
Math or LA and activities are introduced at this time to ensure student
success. The groups are always changing based on students’
ability, interest, or random selection.

Recess Various gross motor activities are set up around the playground
to practice and develop various gross motor skills. Students can
also play on the equipment daily.

Handwashing & Students use the bathroom if needed. All students wash their
Bathroom hands.

Snack In addition to taking in needed nourishments and hydration,
another important aspect of snack time, is the social skills
that are impacted. Students sit with their peers while
teachers demonstrate manners and encourage social
interactions.

Logic or Literature We start with message of the day to building literacy skills.
Then we read a book related to our theme. Students share,
join in, make meaning, and make connections to build reading
comprehension, book knowledge, and/or phonemic awareness.
We may also do connecting activities (charts, graphs, songs,
etc.) to build oral language, social skills, science/math concepts,
and various literacy skills.

Centers Students pick a center they want to work in with a friend or


independently. Students change centers as they wish. Our
centers are art, blocks, discovery, library, pretend, and discovery.
New themed activities are added to each center every week.

At some point during center time, students will work with the

Character Building We focus on building social skills, character, or science. We may


read a book, do a character building or social skills activity, or have
a class meeting.

Dismissal Students will pack up and get ready for their parents/guardians.
This is a great opportunity for preschoolers to gain independent
skills for putting on their own coats, packing up their own bags and
being responsible for their belongings.

Our preschool day has a mix of student lead, play based, and teacher directed lessons
and activities. We infuse movement throughout the day and take a child’s attention
span into consideration as well. This is our daily routine. The routine will not often
change. However, it may be adjusted based on students’ needs, situations (fire drills,
etc.), and classroom events.

Allergies and Medical Conditions


If you preschooler develops any allergy or medical conditions, please
notify your child’s teacher immediately and email:
preschool@mysummitadventures.org

When registering your preschooler, you were asked if your child has any allergies and
medical conditions, your child’s teacher will be notified.

If your student is allergic to nuts or gluten,


please send them with a snack daily.

An allergy sign indicated the classroom is “Nut Free” will be posted in classroom with
nut allergies. When sending treats to school, please choose treats that are nut free.

Class Celebrations
We have several class celebrations and parties; your child’s teacher will send home
notes with dates and details. Some of our celebrations will include:
• Halloween Party
o Wear a costume to school.
o Bring a treat to “trick or treat” with preschool friends
• Thanksgiving Feast “Friendsgiving”
o Teachers will send home a note requesting a food item
• Santa Party
o Santa will come and visit each class.
• Gingerbread Party
o Decorate a gingerbread man
• Valentine’s Party
o Send valentines and a treat to class
• Spring Find
o Send 12 pre-filled plastic eggs
• End of Year Program-Preschool Graduation

More details will come from your student’s teacher.

Super Star and Kudos
Throughout the year each student will be given a chance to be the VIK (Very
Important Kid) in the class. Your child’s teacher will send home more information,
please help your student decorate the poster/paper with pictures and memorabilia.

Throughout the year your child’s teacher will be sending home “kudos” notes letting
you know of a positive behavior and/or action done by your student or a goal that
they have met. Positive Behavior Incentives will be used throughout the year.

Picture Day
Picture day will be offered once in the Fall and once in the Spring. Notes will be sent
home with students with picture pricing and dates. Picture Day’s will be FREE DRESS
DAYS.

My Summit Adventure’s Preschool T-Shirts


Each student will receive a “My Summit Adventure’s Preschool” T-shirt in August.
These T-shirts can be worn at any time, if you would like to purchase more shirts,
they are available for $8.00.

Assessment:
Each student will be an assessment twice a year: Fall and Spring. Teachers will share
the assessment with parents during Parent Teacher Conferences in February.

Parent Teacher Conferences


Parent teacher conferences will be held every February/March. Families will be
notified by email and given the opportunity to sign up for a conference time.

Professional Development Day


Utah State Board of Education offers Professional Development to Preschool
teachers throughout the year. During these professional development trainings days,
there will be no preschool so that our teachers can attend. We will notify parents of
the dates as soon as the Utah State Board of Education posts the dates.

Discipline Policy
Our staff monitors behavior while being aware of the student’s individual needs
and current stage of development. Children are taught socially appropriate behavior,
self-control and respect for others in order to avoid incidents. All students are
provided with an understanding of school rules at an age-appropriate level. Emotional
literacy and conflict resolution skills will be taught at the child’s level so that they can
solve problems independently whenever possible. We teach students to accept full
responsibility for their behavior, their attitudes, their experiences, and their
lives. When possible and necessary, we cue and remind students of appropriate
behaviors.

Disruptive, disrespectful, and/or inappropriate behavior is handled on a case by


case basis. In determining the consequences to be taken, the teachers and
administration take into account the circumstances in which the behavior/incident has
occurred, the harm to others or materials, the child’s age, and the child’s unique set
of circumstances.

At the My Summit Adventures, our goal is to maintain a safe and orderly


environment in which your child can learn. Therefore, we place a great emphasis on
encouraging appropriate behavior of children to help them develop self-control, self-
confidence, and self-discipline. An effort is made to help children understand why some
behaviors are not acceptable, and suggestions for more desirable behaviors are
offered.

The rules are posted in the classroom and reviewed often with children.
Classroom rules children are expected to follow are:
1) Be safe by keeping your hands, feet, and objects to yourself
2) Be respectful and kind to others
3) Be responsible for yourself and your play area
4) Be a good listener especially to your teacher

The following types of discipline and guidance are prohibited:


a. any form of corporal punishment or any action that produces physical
pain
or discomfort such as hitting, spanking, shaking, biting, or pinching.
b. restraining a child's movement by binding, tying, or any other form of
restraint that exceeds gentle, passive restraint.
c. shouting at children.
d. any form of emotional abuse.
e. forcing or withholding food, rest, or toileting; or
f. confining a child in a closet, locked room, or other enclosure such as a box,
cupboard, or cage.

To ensure the safety of all participants and staff, My Summit Adventures staff will
implement a positive discipline program. Children involved in our program are expected
to follow the rules and direction of the program staff. The following are guidelines
used when disciplinary action becomes necessary due to unacceptable behavior.
Severe behavior will be addressed by the Preschool Director/Principal

1. The child will be redirected.


2. The teacher will always talk with the child about the problem and
encourage self-correction.
3. The child will be asked to sit next to the teacher or alone for a while to
work. Think sheet will be Implemented.
4. They may be asked to work in another area where the disruptive behavior
cannot continue until ready to rejoin the group.
5. If the child’s behavior is harmful to self or others, the parent will be called
to pick the child up. Think Sheet Conference with Parents.
6. The school reserves the right to determine whether additional outside
resources need to be utilized. Reoccurring inappropriate behavior will result
in a conference with parents, teachers and administration and the creation
of a support plan/timeline to help the child.

My Summit Adventures reserves the rights to withdraw a participant from the


program if all discipline options have been exhausted and/or demonstration of
extreme behavior that may put participants and staff in danger.

Behavior Contract
(Signed upon Registering)

My Summit Adventure's provides an environment that promotes the social and


academic skills necessary for successful year of preschool. Our teachers are well
trained, and our curriculum is enriched to accomplish this goal. However, we are not
equipped to work with challenging behavior that is not age appropriate nor children
who are demonstrating angry aggressive behavior. Should your child intentionally hurt
another child or show behavior (Examples: Aggressive Behavior, running from teacher
or classroom, creating an unsafe environment for other students or if a student
needs one on one support) that cannot be supported in our preschool class the
following steps will be taken:

1st incident ....................Parents will be notified by phone, Think Sheet will be implemented.

2nd incident ....................Child will sent home from Program, Think sheet will be implemented.

3rd incident ....................An immediate parent conference will be required A behavior plan will
be created

4th incident ....................Child will no longer be enrolled in My Summit Adventure's.

By signing this contract, you are agreeing to support My Summit Adventure's


Preschools high academic and behavior standards and to act as a important partner
is helping your child understand that only appropriate behavior that is acceptable in
our Program.

Photo Release
During the enrollment process you may request to have your child’s likeness not used
in public marketing materials. You authorize the school to make reasonable use of
recordings of school activities in which yourself or your child are involved to train
teachers, increase awareness, and promote continuation and improvement of
programs.

Videos and Slideshows


Throughout the year, videos and slideshows may be shared with you. We hope you
and your child will enjoy watching them. Please keep these items private, they should
not be posted on social media.

Safety Procedures

• Outside doors are locked at all times.


• We ask all staff to be vigilant and watch for people we do not recognize and to ask
anyone we do not recognize how we can help them.
• We have a required check-in for visitors at the front office, where they sign in and
wear a badge notifying our staff of their visitor status.
• Every classroom has an emergency evacuation plan posted. We practice emergency
evacuations throughout the school year.
• We utilize an emergency notification system in the event of a schoolwide emergency
or unannounced school closure.
• We have phones in every classroom by which the office can issue a schoolwide page in
the event of an emergency and with which each classroom can contact the office
immediately in the event of an emergency.
• Our teachers are required to be First Aid/CPR certified.
• At the beginning of the year students receive lessons including a tour of school and
general safety guidelines. These lessons will be reviewed as needed.
• We utilize head counts at various times throughout the school day, particularly when
children are in transition (i.e., when children leave the playground and once, they arrive
back in class).
• Children check in to use the bathroom. If a child does not return within a reasonable
time (age dependent) a teacher will check on them. Children new to the program will
typically have a teacher accompany them to the restroom.

Treats
Throughout the year you will be asked to bring treats or food to the classroom,
please make sure all food brought into the classroom is store bought and sealed in
original container.

Birthday’s
All Birthday’s will be celebrated throughout the year, if your child has a summer
birthday, we will celebrate their ½ birthday. Your child’s preschool teacher will send
home more information regarding birthday celebrations.

Illness Policy
If your preschooler is ill, please keep them home until they feel better.

Colds, flu and other contagious diseases are a serious issue in a school environment
because they can spread so rapidly. Parents are asked to keep children home when
they show symptoms of illness. If your child exhibits any active symptoms of illness,
he/she will not be admitted to the school, both for the child’s own comfort and to
minimize the spread of illness to other children in the school.

How does your child feel? Variations from normal behavior are the best indicators of
illness. You know your child best; trust your instincts.

• Fever. If your child has a fever at night, he/she must not attend school the next
morning. Temperatures are lower in the morning and a fever may occur again in the
afternoon. You are required to wait 24 hours after the fever breaks before sending
your child back to school.
• Upset stomach. If your child vomits during the night, do not send him/her to school
the next day. You are required to wait 24 hours after a child vomits before sending
your child back to school.
• Diarrhea. Loose and frequent stools have many causes. Do not send a child to school
until bowel movements are normal.
§ Cold. Be sure a child knows how to handle tissues for coughing, sneezing and nose
blowing, and practice good hand washing techniques. Your child may go to school as
long as he or she does not have a fever or discomfort. If symptoms are severe (e.g.,
persistent cough or severe runny nose with thick mucous that will consistently
interrupt their work or rest time), please keep your child at home so he/she may
rest and recover.
§ Earache. Never ignore an earache. Contact your physician and keep your child at
home.
§ Strep Throat. A strep infection requires a doctor’s visit and medication. Strep can
lead to a more serious illness if not properly treated. The child must be on medication
at least 24 hours before returning to school.
§ Contagious Condition. Head lice (see below), scabies, impetigo, chicken pox, strep
throat, measles, rubella, mumps, whooping cough, meningitis and some forms of
conjunctivitis (pink eye) are contagious and must be properly treated and no longer
contagious before your child may attend school. Please notify the school if your child
has a contagious condition so that other parents can be If your child has been out of
school due to illness, we ask that you consider whether he or she is well enough to be
outdoors before you send him or her back to school. The outdoors is part of our
program and we do not have the staff to supervise students indoors and outdoors
simultaneously.

Illness or Injury at School

If a child becomes ill at school, A parent/guardian will be contacted and will be


expected to pick up the child as soon as possible. If a child is injured at school, first aid
will be administered if the injury is minor. An accident report will be filed for injuries
which require medical attention, including first aid. Parents will be asked to sign the
accident report when they pick up their child and will be notified immediately if there
are any questions concerning the severity of the injury. The student’s emergency
contacts and physicians will be called if the parents are unavailable. All classroom
teachers are CPR/First Aid certified. Paramedics will be called when necessary.

Connecting with the Classroom


We encourage open and ongoing communication between home and school. Our
teachers have office hours so that you can meet if you have any questions,
comments or concerns regarding your child, the classroom, or their program in
general. Office Hours are announced annually.

Each classroom has an email address that reaches the team for that class. In
addition, each teacher has an individual email for person specific items. Teachers are
typically unable to reply to emails or take phone calls during class time as their
responsibility is to the students, they will respond during their planning time.

Each Class will use the “remind” app to increase parent communication. Please visit
our website for each classes individual code.

I, acknowledge that I have read the Preschool Handbook and are aware of the
policies and procedures at My Summit Adventures Preschool. Those policies include the
following important highlights:

• Children will not be allowed to leave the school with any person that is not on the
emergency contact list or who does not have a visor card provided by the school.
• Snacks will be provided daily by the preschool staff and the parents are responsible
to disclose any food allergies or dietary needs to the Preschool Director and/or the
child’s teachers.
• Teachers are not permitted to help any child in the restroom other than buttoning
and zipping pants out in the hallway. If there is a need for adult help, the parents will
be called.
• Students may be picked up in the loading area from 11:20-11:30 for AM classes and 3:10-
3:20 for PM classes. If a parent arrives after the outlined times, students may be
picked up at in the classroom. Parents will be charged $1 per minute after the time
indicated above. This amount will be put on the following month’s bill.
• Each monthly payment is due between the first (1st) and tenth (10th) of each month.
Checks should be payable to My Summit Adventures Preschool. A $25 late fee will
apply after the tenth (10th) of the month. Tuition that is not paid by the 20th of the
month will result in a DO NOT ATTEND. Failure to pay tuition as scheduled may result in
your student losing his/her place at the preschool.
• To withdraw your student, a 30-day written notice must be given to the Preschool
Director.
• Parents must provide current immunization records to the preschool and have
turned in signed consent/release and agreement forms before the student begin
attending class.
• For the safety of everyone involved, drivers must become familiar with the
loading/unloading procedure and may not get out of the car at any time while in the
loading lane. Drivers must pull over to the left if a child needs help buckling.
• Teachers are not permitted to buckle students into the vehicle.
• My Summit Adventures Preschool has the right to decline continued enrollment to
any student who is or has parents that are, uncooperative or abusive to school
administrators, staff, or other students. Abuse includes, but is not limited to, biting,
kicking, spitting, and verbal abuse or violation of the behavior contract.

Media Consent

Student Media Consent and Release Form Throughout the school year, students may
be highlighted in efforts to celebrate their achievements and promote My Summit
Adventure's activities, programs and achievements. This includes internal and external
media such as social media, district’s website and other media platforms.

As the parent or guardian of my student, I grant permission to My Summit


Adventures and its School Board, agents, employees and/or school staff to
photograph my child and to record voice, performance, poses, acts, plays and
appearances. I further grant my child’s school all rights to use these sounds, still, and
moving images and other reproductions of physical likeness in any medium (i.e. print,
website, video, social media). I understand that neither My Summit Adventures nor its
representatives will reproduce said photographs, interviews or likeness for any
commercial value or receive monetary gain for use of any reproduction/broadcast of
said photographs or likeness. I am also fully aware that I will not receive monetary
compensation for my child’s participation. I further release and relieve My Summit
Adventures, its School Board, employees and other representatives from any
liabilities, known or unknown, arising from the use of this material. I understand that I
am free to address any specific questions regarding this release by submitting those
questions in writing prior to signing to the My Summit Adventures Student Media
Consent and Release Form. I agree that my failure to raise any questions in writing in
conjunction with the signing of this document will be interpreted as a free and
knowledgeable acceptance of the terms of this release.

Consent, Waiver, and Release of Liability

I hereby consent to allow my child to attend My Summit Adventures and participate


in all activities sponsored by My Summit Adventures. I represent that my child is
properly immunized against all childhood diseases or illnesses.

I represent that I have advised My Summit Adventures staff regarding any allergies
or physical disabilities that my child may have. I agree to indemnify and hold My
Summit Adventure's its directors, employees, and agents harmless against any and all
claims arising from injury or illness my child may suffer as the result of food allergies
or disabilities.

I represent that my child has health insurance coverage and I agree that I will
maintain said coverage during the entire time my child attends or participates at My
Summit Adventure's. I have been advised that My Summit Adventures does not
provide health insurance or medical coverage to my child in any form under any
circumstances.

I agree to indemnify and hold My Summit Adventures its directors, employees, and
agents harmless against any and all claims arising as a result of my child attending
and/or participating in the activities of My Summit Adventures.

I authorize My Summit Adventures and its directors, employees, and agents to


provide first aid care, or emergency medical care to my child and/or to obtain such
emergency medical care for my child as may appear reasonably necessary in my
absence including emergency transportation to a hospital. I agree to be financially
responsible for any and all medical expenses or costs that are incurred in treating my
child for illness or injury when said illness or injury arises as while my child is on the
premises of or in the custody of My Summit Adventures.

I CERTIFY THAT I HAVE READ THE ABOVE AGREEMENT AND AGREE TO THE TERMS
THEREOF.
