Health Aid Connect is a non governmental organization that helps people in need by giving
them essential health supplies. We want to make sure that everyone can get the basic things they need
for good health by providing healthcare kits.Through our organization, HealthAid Connect aim to foster
a doings that shows support in enhancing the well-being and of individuals in the family.

What is the project all about?

VitalAssist by Empowering Wellness is a project that gives important health kits to people in
faraway or mountainous areas where it's hard to find medicine. The goal is to make sure that people in
these places can quickly and easily get the medicine they need, promoting good health in areas with not
many healthcare options.

How do you know that the project is needed?

The "VitalAssist: Empowering Wellness" project is important because it helps people in faraway
or mountain areas where it's hard to get medical help. By giving essential health kits, the project makes
sure that people in these places can quickly and easily get the medicine they need. This is crucial for
those people who want to have a improvements in health in areas with not many healthcare options,
only the herbs. HealthAid Connect is making a positive impact on the lives of those who struggle to get
the medical support they need.

Problem Statement:

Rural Community areas often experience barriers to healthcare such as distance,

transportation, cost and insurance. We need to offer first aid prevent simple injury become severe and
in some case life-threatening as a result of lack of immediate medical treatment.

Vision:

To connect to the hearts of people and deliver health love in a box for future needs.

Mission:

To inspire faith and contribute to the health of people by providing the best care.

1. Purpose- why do you want to do that particular service; what need does the community have
for this particular service?

The purpose of this particular service is to reach out essential health kits to rural areas
who have experienced barriers towards medical facilities. This will benefit the families in
the community to have supplies whenever in need and avoid simple wounds becoming
severe injuries that could lead to life fatality.

2. Community in which you plan to do the project; service population ( children, elderly, pre-teens,
adolescents, homeless, rural underserved, chronic disease management, etc.) why did you
select this particular community?

3. What do you expect to learn, or gain, as a result identifying , planning, developing, and
implementing your ideas?

There are many learning we gain in planning and implementing this project. Through
this, we will be able to learn necessary skills and knowledge that will certainly benefit us
in our future careers. Those skills are effective communication, the ability to pursued
and explain clearly our mission to our funding sources and benefeciaries. The ability to
handle pressure and solve problems as we face many challenges during the
implementation of the project. Also, the ability to conduct a good plan in any projects
we will implement in the future

4. Timeline- from start to completion, how long do you expect this project to take? How might the
service/project be divided up into do-able mini projects to make it feasible and to ensure
ultimate success?

This project will only take one day for implementation but it really need some time in
gaining the information and resources we need for this project. To ensure that this
project attain success, we make sure that we gain all the significant information like the
number of residents of the baranggay where our project will be conducted and some
investigations about the baranggay if our said projects is necessary to that baranggay.
We also need time to solicit funds on some organizations for our project.

5. What are you going to do? Describe the plan for your time and effort?
Our team decided to conduct a project in regards to the health of the people. Thus, we
come up with the idea of giving medicine Kits to the residence of a baranggay. We
choose some medicine that need by many people like paracetamol and we also added
alcohol, betadine and band aid incase of emergency situations like injury in which the
residents will be able to access those things quickly. Furthermore, we ask many
pharmacies about the price of the medicines thus, we realize that we need huge
amount of fund. So, we ask our relatives if we will be able to request help of some
organizations like SK

source?

8. What are the lasting results of your idea/project for the community in which you plan to do?
For the results that we are expecting is for the supplies to be properly used and be given value
to the people in the community.
Our project might only be implement in one day but we have our goals in this said
project; Implement awareness of the importance of having medicine kits. To put in mind that
this project was not just giving of medicine kits but also giving knowledge of the importance of
having those kits. We will awaken their mind that the fact we can't predict the future so we
must prepare for any situations.
