Mission Statement
To ensure health and well-being for all, to provide access to safe and effective medical
sessions and to achieve universal health coverage.
Vision Statement
A community in which all people achieve their full potential for health and well-being
across the lifespan. We work to be trusted by patients, a valued organization in the
community, and creators of positive change.
Currently, the world is facing a global health crisis unlike any other —
COVID-19 is spreading human suffering, destabilizing the global economy
and upending the lives of billions of people around the globe. Before the
pandemic, major progress was made in improving the health of millions of
people. Significant strides were made in increasing life expectancy and
reducing some of the common killers associated with child and maternal
mortality. But more efforts are needed to fully eradicate a wide range of
diseases and address many different persistent and emerging health issues.
By focusing on providing more efficient funding of health systems, improved
sanitation and hygiene, and increased access to physicians, significant
progress can be made in helping to save the lives of millions. Health
emergencies such as COVID-19 pose a global risk and have shown the critical
need for preparedness and awareness so we are also here to take actions to
bring about health benefits by raising awareness in communities for
prevention and to promote healthy lifestyles. It is better if our bodies are in
good shape, we can do amazing things. Everyone wants to be healthy and live
along happy life that is why having a good health and well being are very
important and not to be ignored.

Project Details
a. Deliverables
Our project wants to end the spreading of the virus and all the epidemics
in deprived areas, reduced the maternal mortality rate and eradicate the
preventable deaths. We want activities covering mostly referrals from our
barangay health stations and preventive, promotive and curative aspects of
health care. The barangay health station is the initial unit, which dispenses
basic health care, i.e. maternal and childcare, immunizations, treatment of
simple medical conditions, nutrition, family planning, sanitary health care,
emergency treatment and health education to provide a healthy life and well
being for all people of all ages.
b. Target Beneficiaries
The target beneficiaries of the project would be the people of all ages but
more specifically those people in rural area and other deprived areas because
they are associated with worse health care assistance because of lack of social
resources.
General Timeline
The estimated fund for this project will cost P40,000,000. The 60% of this
amount would be allocated to the cost of buildings construction and the
remaining 40% of this amount would be allocated to the appliances,
furnitures and other things that are needed in our facilities.
There are different ways that we could evaluate this project and this
include things like collecting information on the achievements of the
operation and comparing it with the initial objectives of this project. It’ll be
like feedback from stakeholders and from the project team members asking
the questions like What went well?, What could have been done better?,
Where were the actions different to the plan? And why those things
happened. We can also employ an independent evaluator to take on this part
of this project so they would offer us an outside eye on what we are doing
when we were undertaking evaluation of this project.
