Energy Requirements
• Neurons and cardiac myocytes share the
property of conduction of electrical impulses.
• Neurons are highly dependent on aerobic
metabolism because they must use this
energy to maintain proper ion gradients.
• Inhibition of aerobic respiration (e.g.,
cyanide) or hypoxia (e.g., CO poisoning)
leads to early sign of neuronal dysfunction.

Mechanisms of neurotoxicity (IV)
• Astrocytes(星形膠質細胞,也稱星狀細胞,為中
樞神經膠質細胞的一種。星形膠質細胞執行著許多
功能,包括形成腦血管障 壁 (BBB) 的內皮細胞的

Summary
• The central nervous system (CNS) is protected from the adverse
effects of many potential toxicants by an anatomical blood-
brain barrier and by the action of membrane transporters.
• Neurons are highly dependent on aerobic metabolism because
this energy is needed to maintain proper ion gradients.
• Individual neurotoxic compounds typically target the neuron,
the axon, the myelinating cells, or neurotransmitter system.
• Neuronopathy is the toxicant-induced irreversible loss of
neurons, including its cytoplasmic extensions, dendrites and
axons, and the myelin ensheathing the axon.
• Neurotoxicants that cause axonopathies, cause axonal
degeneration, and loss of the myelin surrounding that axon;
however, the neuron cell body remains intact.
• Numerous naturally occurring toxins as well as synthetic
chemicals may interrupt the transmission of impulses, block or
accentuate trans-synaptic communication, block reuptake of
neurotransmitters, or interfere with second messenger systems.

鴉片類
鴉片類(Opiates)與類鴉片類(Opioids)
鴉片類及合成鴉片類藥物過量致死
• 致死主因: 呼吸抑制
• Opioid use disorders 仍是世界最嚴重成癮問題:美國類
鴉片類危機(Opioid Crisis or Epidemic)為例,1990年代
起,美國 因為部分藥廠向醫事人員促銷並保證,其所生
產的類鴉片類 止痛劑(Opioid pain relievers)成癮性不高,
造成 oxycodone (商品名OxyContin)等類鴉片類止痛劑
大量誤用、 濫用與流用,其後美國開始管制,使成癮者
又轉向海洛因。 光是2017年一年,就有超過47000人美
國人死於類鴉片類過量 ,類鴉片類的流行更使美國的平
均餘命從2014年的78.8歲連 續幾年降低到2017年的78.5
歲,這在先進國家是很少見到的 ,顯示出問題的嚴重性
• 類鴉片類的作用與毒性較天然的嗎啡有過而無不及:例如
芬太尼Fentanyl: 嗎啡 50-100倍,卡芬太尼Carfentanil

古柯鹼之毒性
◼ 成癮性
◼ 慢性中毒
• 消化障礙、食慾不振、流涎、消瘦。
• 中樞神經系統變性。
• 長期使用後有失眠、震顫現象,時有發生痙攣、幻想、精神
錯亂、譫 妄及感覺與運動障礙。
◼ 急性中毒
• 興奮、不安、多話、焦慮與錯亂、頭痛、脈搏加速、呼吸不
規則變化 。
• 發高燒、瞳孔散大、眼球突出。
• 噁心、嘔吐、腹痛。
• 大量注射易因心律不整、心肌梗塞及心臟衰竭而致死
◼ 對懷孕婦女的危害
• 母體血壓升高並促進子宮收縮,造成流產或早產。
• 減少通過胎盤之血液及氧氣,致使胎兒發育延遲。
• 嬰兒體重較輕、身高較矮、頭圍較小。

大麻濫用型態
• Marijuana :通常製劑形式煙捲,每支約含 5~
20mg THC
• Hashish :THC含量高達其重量之 25 ~60%
• Hash Oil :含更高濃度之 THC

大麻 Cannabis
◼ 大麻的活性成分非常複雜,主要存在於其雌花序
中。
◼ 目前分離出來的大麻素超過130種,最主要的活
性物質為:
➢ 四氫大麻酚 (Tetrahydrocannabinol (THC )
➢ 大麻二酚(Cannabidiol,CBD)
• 大麻成分藥物Epidiolex 上市,這種藥物用於治療2 種
罕見且嚴重的兒童癲癇症 Lennox-Gastaut syndrome
/Dravet syndrome

大麻的危害
◼ 急性毒性
➢ 高劑量易產生妄想、精神疾病
➢ 低專注力,影響思考
➢ 短期記憶受損。
➢ 運動協調受損: 例如開車易造成意外事故。
◼ 慢性毒性
➢ 改變大腦發育:青少年的大腦尚在發育階段,影響程度最大。
➢ 損害肺功能,慢性支氣管炎
➢ 動物實驗,殘留致癌物質benzo[a]pyrene含量高於香煙。
➢ 造成生育能力受損。
➢ Psychosis and Schizophrenia 。
➢ 低動機或無動機(Amotivation):生活滿意與成就感較低。
◼ 大麻戒斷症狀: 身體緊繃、睡眠問題、食慾不佳,熱潮紅、疲
倦、盜汗、焦慮、憂鬱、情緒起 伏大

安非他命類
◼ 安非他命:1887年,由鹽酸麻黃素合成。
◼ War Effect: 二戰時,德軍及日本神風特攻隊使用。
◼ 濫用方式:口服、注射或鼻吸或與煙草混合後抽吸
◼ 甲基安非他命鹽酸鹽結晶如細碎冰塊,匿稱冰塊
(ice)。

安非他命濫用之臨床症狀
• 安非他命的濫用會造成兩大類的後果:
使用疾患,包括濫用 (abuse) 與依賴 (dependence),
藥物引起的身體疾患,包括中毒 (intoxication)、戒斷
(withdrawal) 等。神經及精神系統方面-安非他命精
神病 (Amphetamine psychosis)。
• 「執著」(Punding)行為: 慢性的、無目的的刻版行為,
常見於長期安非他命使用者 ,此現象為瑞士的司法精
神科醫師G. Rylander於1968 發現 ,又稱為Dopamine
dysregulation syndrome (DDS)
• 心臟血管系統方面:心跳過速、血管收縮、血壓上升、
肺水腫、心衰 竭。
• 體溫調節異常及其它器官之併發症。
• 死亡原因:心衰竭、腦血管病變、橫紋肌溶解
(rhabdomyolysis)、急性腎衰竭、精神症狀發作導致
意外事故。

何謂新興毒品?
• 新興毒品或新興影響精神物質 (New Psychoactive
Substances, NPS)定義: 聯合國毒品與犯罪辦公室
(UNODC) 使用 new psychoactive substances
(NPS)”這個專有名詞,係 指 濫用的物質沒有被
1961年「麻醉藥品單一公約」或 1971年 「影響精
神物質公約」所列管,但是其濫用會在某期間造成
公共衛 生威脅者
• 所謂的 新 不一定是新發明,有些NPS是在40多年
前被首先 合成,只是到近幾年才造成濫用,因此沒
有被上述聯合國公約 所列管

NPS 分類
• Synthetic cannabinoids (合成大麻素)
• Synthetic cathinones (合成卡西酮)
• Ketamine(愷他命) 及 Phencyclidine (PCP)
• Piperazines(哌𠯤)
• Phenethylamines:除安非他命類(ATS)外,多未被列管
• Plant-based substances(植物性成癮物質):例如 Kratom,
Salvia, Khat Tryptamines(色胺)
• Aminoindanes(氨基茚滿類): sold as NPS for their
ability to produce empathogenic and entactogenic
effects of serotonin releasing drugs, such as MDMA
• Others:無法歸類於上述者:,包括Classic hallucinogens
(psychedelics)、 Opioids 、Sedatives / Hypnotics、
Stimulants

NPS的毒性問題
• 依照藥理作用,和傳統毒品類似,可以分為興奮、
抑制與迷 幻作用。但是由於多未經研究,劑量、
毒性和副作用無從預 防。
• NPS常有多種混用情形,使用之後果難以預測。
• 毒咖啡包

搖頭丸(MDMA)/強力搖頭丸(PMMA)
1.造成腦中血清素和正腎上腺素大量釋放,再加
上抑制血清素和正腎上 腺素代謝, 毒性較強。致
死劑量和產生作用劑量接近
2.藥物起效慢,導致使用者常追加劑量而中毒。

低劑量
• “醉意 、暈眩,似夢般思考
• 感覺延遲或降低,手腳不靈活,肢體笨拙或機器人動作
• 有時有欣快感
• 社交性增加
• 覺得可以以不同視野看世界
高劑量
• 短暫性癱瘓、移動困難
• 噁心、嘔吐
• 完全解離
• 瀕死經驗
• 失神
• 強直痙攣
• 昏迷、死亡

Ketamine 濫用者各器官的疾病
◼ 心臟毒性 (Cardiotoxicity): Tachycardia,
hypertension
◼ 泌尿道毒性(Urological Toxicity) Interstitial cystitis,
bladder atrophy患者一開始以下泌尿道症狀為主,之
後 發生萎縮性膀胱合併尿容量減少與雙側腎水腫,甚至
進展成末期腎病變 。
◼ 胃腸道毒性(Gastrointestinal toxicity): K-cramps”位
於上腹部或右上腹的鈍痛)可能同時合併總膽管擴張及
肝功能異常
◼ 認知損傷 (Cognitive impairment) Memory deficits
Executive function
◼ 精神疾病 (Psychosis) K-hole”; NMDA model of
schizophrenia
◼ 成癮 (Addiction)
