1. With reference to your Salary Account offer dated __________ for opening salary accounts of our employees, we are
pleased to inform you about our acceptance of the offer, inclusive of all the terms and conditions mentioned therein. (The
offer is subject to review based on the above criteria after a time period of 6 months)

2. We shall inform our employees to submit their Axis Bank Salary Account number to our relevant department. We
understand that in case of non-credit of salaries for more than 3 consecutive months, the special features offered under the
respective salary schemes would stand withdrawn and the account shall be treated as a normal savings account under the
Bank’s standard charge structure and all charges shall be levied as applicable to normal savings accounts.

3. In case of any wrong details being provided by us to the Bank in terms of individual employee salary account number / net
amount to be credited for the employee, we confirm that the Bank is not liable for any losses caused therein.
