JERSEYS LOAN AGREEMENT

This agreement is between MALAYSIA ICE HOCKEY FEDERATION (hereinafter referred to as “MIHF”) and
___________________________________(IC NO: ______________________) (hereinafter referred to as “the Player”).

1. Terms and Conditions of Jerseys Loan Agreement: -

1.1. MIHF will lend the team national jerseys (hereinafter referred to as “the said Jerseys”) to the Player on
the terms and conditions as stipulated in this agreement.
1.2. The said Jerseys shall be loaned to the Player for the following tournament:
1.2.1 Name of the tournament : IIHF WORLD CHAMPIONSHIP MEN DIVISION IV
1.2.2 Venue of the tournament : KUWAIT
1.2.3 Date of the tournament : 16TH – 19TH APRIL 2024

2. The Player’s Responsibilities

By executing this Agreement, the Player agrees to comply with the terms set forth below: -
2.1 The Player is responsible for the full cost of repair or replacement of the said Jerseys that is damaged,
lost, confiscated or stolen from the time the Player assumes custody until it is returned to the MIHF. If
the said Jerseys are lost, stolen or damaged, the Player agrees to promptly notify the MIHF’s
representative.
2.2 The Player agrees to use the said Jerseys for the purpose of training and tournament approved by the
MIHF only and not for any commercial use or application unless with permission in writing from the MIHF.
2.3 The Player shall be responsible for the safe keeping of the said Jerseys and the said Jerseys shall be
returned to the MIHF within SEVEN (7) days after the tournament ended.
2.4 The said Jerseys shall be returned by the Player to the MIHF in a good condition as when received by the
Player, except for reasonable wear and tear. During the said loan period, the Player agrees to assume all
responsibility for maintenance and repair of the said Jerseys.

3. Amendment/Modification
This Agreement cannot be amended or modified except in writing and signed by both parties.
