AGREEMENT TO WAIVER OF CLAIMS AND LIABILITY RELEASE

1. Please note that in signing this agreement, all players must understand that there is NO
insurance coverage provided in respect of this competition and, to be insured, the player
must arrange their own separate cover with an insurance provider or broker.
2. No player will be allowed onto the field unless he/she has signed the liability/waiver form
indicating his/her acceptance of the rules.
3. By signing this liability/waiver form I accept all risks associated with my participation in the
GFC Football Tournament.
4. The team MANAGER is responsible for interpreting this AGREEMENT TO WAIVER
DECLARATION to his players.
