Planning

- After diagnosis, the next step is planning the interventions that will be used
to solve the specific problems as stated by the patient. In King’s goal
attainment, the planning part of the nursing process is represented by setting
and deciding about the goals and the means to achieve them to be agreed
upon by the nurse and the patient in order for a transaction to occur.

Identifying the goals and planning the means to achieve these goals

What goals do I think will benefit the patient’s best interest?

1. The client will experience improved comfort as indicated by:

 a decrease in pain due to abdominal cramps and headaches


 Being able to rest and sleep comfortably

2. The client will be free from infection as evidenced by normal body


temperature and normal vital signs.

3. The client will have improved bowel elimination as indicated by:

 Stool becomes less frequent and watery

4. Client will gain sufficient knowledge regarding treatment and home care.

5. Client will attend to heal problems promptly.

What are the patient’s goals?

Patient’s goals are:

 Freedom from pain


 Improved bowel movement
 Acquisition of sufficient knowledge regarding his health problems

Goal 1:

 Assess characteristics of pain


 Continuous administration of medicine as prescribed by physician
 Provide a comfortable resting position as requested by client

Goal 2:

 Monitoring vital signs and body temperature after each bowel movement
 Keeping anal area clean after each bowel movement

Goal 3:

 Ensure that the client’s diet consists of foods rich in potassium and avoid
the intake of products containing caffeine or milk and/or any dairy
products.
 Ensure that the client has adequate fluid intake

Goal 4:

 Explain the treatment measures to the client in a simple and understable


manner.
 Explain home care by demonstration
 Clarification of any doubts of the client that he presents are of importance
 Repetition of information as needed to reinforce learning

Goal 5:

Health education given about the following:

 Proper hand washing after each bowel movement and prior to preparing
food for himself and/or for others
 Rehabilitation measures to promote a healthier lifestyle

 Pain can be tolerated by the patient and requires no medication
