Dear Rishabh Lamba,

This has reference to your application and subsequent interview with us. We are pleased to offer you the
position of Commis 1, Grade — TL 9 at Shivoy Hotel, Gorakhpur.

This offer is valid subject to your clearing the medical examination and submission of the police
verification report and joining latest by July 04, 2017 at 1000hrs.

As discussed, your annual salary (CTC) shall be 2,88,000/- (Two Lakhs Eighty Eight Thousand Only).
You will also be covered under the Mediclaim and Personal Accident Insurance Policies as per Company
rules. Which is going to paid to you through cash mode

On the date of your resuming duty at our office, you may please bring along the following: -

1. Proof of Age.
2. Copies of educational certificates.
3. Copies of experience certificates.
4. Relieving certificate from the previous employer.
5. Appointment letter and salary revision letters of the previous employer.
6. Last pay slip received from the previous employer.
7. Form 16 (TDS Certificate), if any.
8. Proof of investment/savings for income tax reduction.
9. 5 Passport Size Photographs.
10. 2 References.
11. 2 Aadhar Card copy (Mandatory).
12. 2 Pan Card copy (Mandatory)
13. Cancelled cheque/ Passbook copy of existing bank account (Individual saving account)

You will be issued a Letter of Employment with all the applicable terms and conditions on your date of
joining.
